宫颈冷刀锥切辅助宫腔镜下电凝止血术联合美宝湿润烫伤膏治疗CIN30例临床分析

摘　　要：目的：探讨宫颈冷刀锥切辅助宫腔镜下电凝止血术以及术后局部应用美宝湿润烧伤膏在CIN的诊断和治疗中的价值。方法：30例患者均为在本院经宫颈液基薄层细胞学（TCT）检测、阴道镜、宫颈活检三级筛查确诊为CIN,于月经干净3~7 d内手术,并切下标本做标记送病理。术后第2天于宫颈创面涂擦美宝湿润烫伤膏7 d。结果：无一例发生术中大出血。阴道镜下多点活检与锥切术后病理结果诊断完全相符者25例,占83.3%,不符者5例,占16.7%。其中有2例程度较轻（6.7%）,3例（10.0%）锥切术后病理检查较阴道镜下多点活检程度重,CINⅢ2例,（术前诊断为CINⅡ）,CINⅡ1例（术前诊断为CINⅠ）。结论：阴道镜下多点活检对于CIN患者术前的诊断价值较高,且宫颈冷刀锥切辅助宫腔镜下电凝止血联合美宝湿润烧伤膏治疗可有效控制出血,值得临床推广应用。Objective：To investigate the cervical cold knife cutting auxiliary cone blood an electrocautery unit under hysteroscopy surgery and postoperative local application Mabel moist burn cream value in the diagnosis and treatment of CIN.Method：30 patients were in our hospital after cervical liquid thin-layer cytology test（TCT）,colposcopy,cervical biopsy diagnosis of CIN,tertiary screening in menstrual clean surgery,3-7 days.Cut the specimen marking to send pathology.Gauze were harvested at the next day,the cervical wound inunction Mabel moist burn ointment for 7 days.Result：No intraoperative bleeding in 1 case.Multi-point biopsy under colposcope diagnosis with taper cutting postoperative pathologic results correspond exactly 25 cases,accounted for 83.3%,was not in conformity with the 5 cases,accounting for 16.7%.There were 2 cases to a lesser degree（6.7%）,3 cases（10.0%） of the cone cutting degree of postoperative pathologic examination was the multi-point biopsy under colposcope,CIN Ⅲ 2 cases（preoperative diagnosis for CIN Ⅱ）.CIN Ⅱ in 1 case（preoperative diagnosis of CIN Ⅰ）.Conclusion：The diagnosis valur of multiple point colposcopic biopsy in patients with CIN is higher,and the cervical cold knife conization electrocoagulation auxiliary under hysteroscope combined with Mabel moist burn ointment can effectively control the bleeding,so it is worthy of clinical application.
