手助腹腔镜与开腹手术治疗结直肠癌疗效对比的Meta分析

摘　　要：目的应用Meta分析评价手助腹腔镜(hand-assisted laparoscopic,HALS)与开腹手术(open surgery,OS)两种术式治疗结直肠癌的疗效及安全性。方法检索筛选发表于2000年至2013年的有关手助腹腔镜与开腹手术两种术式治疗结直肠癌疗效比较的文献,使用Rev-Man 5.0软件进行数据处理。结果共纳入了8篇文献,计812例患者,其中手助腹腔镜手术治疗407例,开腹手术治疗405例。Meta分析结果提示手助腹腔镜组出血量少(P<0.000 01),肠功能恢复时间早(P<0.000 01),术后住院天数少(P<0.000 01),术后并发症发生率较低(P=0.009);而手术时间及清扫淋巴结数目与开腹组比较,差异无统计学意义。结论手助腹腔镜手术在结直肠癌治疗上较开腹手术更加安全有效。Objective To compare outcomes of hand-assisted laparoscopic and open surgery radical resection of colorectal cancer using Meta analysis. Methods Retrieved clinical trials comparing hand-assisted laparoscopic with open surgery radical resection of colorectal cancer published from 2000 to 2013. Data were analyzed by Rev-Man 5. 0 software. Results 8 clinical trials were included in this analysis. These studies included a total of 812 patients： 407 treated with hand-assisted laparoscopic and 405 treated with open surgery. The Meta statistical showed as follow： hand-assisted laparoscopic was more less in surgical blood loss（ P 0. 000 01）,earlier in bowel function recovery time（ P 0. 000 01）,less in post-operative stay in hospital（ P 0. 000 01）,slightly lower in postoperative complications（ P = 0. 009） than open surgery. There were no statistical differences between the two techniques in the operation time and the number of lymph nodes. Conclusions For the treatment of colorectal cancer,compare with open surgery,hand-assisted laparoscopic is more safe and effective.
