非小细胞肺癌患者外周血循环肿瘤细胞的检测及与病理分期关系

摘　　要：目的探讨非小细胞肺癌患者外周血循环肿瘤细胞(CTCs)检测的临床意义。方法采用CellSearch系统检测30例非小细胞肺癌患者外周血中循环肿瘤细胞数,并与15例肺良性疾病患者、30例健康志愿者(对照组)进行比较。结果CTCs>0的非小细胞肺癌患者为20例(66.7%),肺良性疾病患者为4例(26%),健康志愿者为0例;CTCs>5的分别为:9(30%)、0、0例;CTCs>10的分别为:4(13%)、0、0例,均有统计学差异(P<0.05)。不同临床分期患者比较,Ⅰ、Ⅱ期患者外周血中的CTCs的阳性百分率(29.6%)显著低于Ⅲ、Ⅳ期患者(58.3%),差异有统计学意义(P<0.05)。结论采用CellSearch系统,可以早期检测非小细胞肺癌患者外周血CTCs,有望成为一种实用的循环肿瘤细胞检测技术。Objective To investigate the expression of circulating tumor cells （CTCs） in peripheral blood cells in patients with non-small-cell lung cancer （NSCLC） and evaluate its clinical significance. Methods The peripheral blood CTCs were detected in 30 patients with NSCLC, 15 patients with benign lung diseases and 30 healthy controls by CellSearch System. Results In the group with CTCs 〉0, the patients with NSCLC and lung benign diseases accounted for 66.7% （n =20） and 26% （n =4）, and the percentage of the healthy controls was 0% （ n = 0）. In the group with CTCs 〉 5, the counterparts were 30% （ n = 9 ） ,0% （ n = 0） and 0% （n =0） ; in the group with CTCs 〉 10, that were 13% （n =4） , 0% （n =0） and 0% （n =0） , respectively. All the outcomes showed significant differences （ P 〈 0.05 ）. According to the different clinical stage of NSCLC, the positive rate of CTCs in stage I / II （29.6%） was lower than that in stage III/IV （58.3%） significantly （P 〈 0.05 ）. Conclusion Detecting CTCs in NSCLC patients by CellSearch System contributes to early clinical treatment of NSCLC.
