焦虑症患者客体旋转的正镜像差异及事件相关电位P300研究

摘　　要：目的探讨焦虑症患者客体旋转的脑电生理机制。方法对来自第四军医大学西京医院的20例焦虑症患者和24例健康被试进行客体旋转任务的事件相关电位(event-related potential,ERP)测定。测量P300的潜伏期、波幅及其错误数和反应时。结果与对照组相比,焦虑症患者组正像和镜像潜伏期延迟[正像:(320.15±32.14)ms vs(362.66±33.62)ms;镜像:(329.18±30.25)ms vs(364.43±35.37)ms]、波幅降低[正像:(13.36±2.23)μV vs(6.49±3.56)μV;镜像:(9.62±2.84)μV vs(5.46±2.51)μV],差异均有统计学意义(P<0.001)。两组潜伏期正像与镜像均差异无统计学意义(P>0.05);患者组波幅正像与镜像差异无统计学意义(P>0.05),对照组正像波幅高于镜像,差异有统计学意义(P<0.001)。结论焦虑症患者心理旋转能力明显受损,其正镜像差异性消失,可为其早期临床诊断提供客观辅助指标。Objective To explore the brain electrophysiological mechanism of object rotation in anxiety disorders. Methods Twenty patients with anxiety disorders and 24 normal healthy controls from the Xijing Hospital, Fourth Military Medical University, performed the mental rotation tasks. The incubation period and amplitude of P300 and the wrong number and the reaction time were measured. Results Compared with control group,the latencies of normal and mirror were significantly delayed in anxiety group[ normal：（320. 15 ± 32.14）ms vs （362.66 ±33.62）ms,P 〈0.001 ;mirror： （329.18 ±30.25）ms vs （364.43 ±35.37）ms,P 〈0.001 ] ,and the amplitude was significantly decreased [ normal ： （ 13.36 ±2.23 ） μV vs （6.49 ± 3.56 ） μV, P 〈 0. 001 ; mirror ： （9.62 ±2.84） μV vs （ 5.46 ±2.51 ） μV, P 〈 0.001 ]. The latency was not significantly different between mirror and normal in two groups （ P 〉 0.05 ）. The amplitude was not signifi- candy different between mirror and normal in anxiety group （ P 〉 0.05 ） ,while the amplitude of normal was significantly higher than that of mirror in control group （ P 〈 0. 001 ）. Conclusion The mental rotation ability of anxiety disorders is impaired, and the mirror-normal difference in mental rotation disappear. P300 can be used as an early-stage clinical auxiliary diagnostic index.
