联合检测血清GP73,AFP,AFP-L3及AFU在原发性肝癌诊断中的作用

摘　　要：目的探讨联合检测血清高尔基体糖蛋白GP73,AFP,AFP异质体(AFP-L3)以及α-L-岩藻糖苷酶(AFU)在肝癌诊断中的意义。方法收集原发性肝癌81例,肝硬化72例,慢性乙型肝炎28例,健康对照27例,分别检测每例的血清GP73,AFP,AFP-L3及AFU,并对统计结果进行分析。结果单项检测中AFP-L3特异度最高为88.8%,AFP灵敏度、准确度最高分别为60.5%、73.2%,而联合检测组中,特异度、灵敏度、准确度分别为89.2%、60.5%及77.03%,高于各单项检查组;ROC曲线分析提示联合检测组的线下面积最大为0.765,大于各单项检测组;联合检测的阳性检出率与各单项检测相比,均高于单项检测,有统计学差异。结论联合检测血清血清GP73,AFP,AFP-L3及AFU有助于提高原发性肝癌的检出率,并与肝硬化、慢性肝炎相鉴别。Objective To investigate the combined detection of serum Golgi glycoprotein GP73,AFP,AFP heterogeneity（ AFP-L3） and α-L-fucosidase（ AFU） in the diagnosis of hepatocellular carcinoma. Methods 81 cases of primary liver cancer,including cirrhosis 72 cases,chronic hepatitis B 28 cases,healthy controls 27 cases,were detected in serum GP73,AFP,AFP-L3 and AFU,and statistical analysis was made of the results. Results Single test result showed that highest AFP-L3 specificity was 88. 8%,highest AFP sensitivity and accuracy were up to 60. 5%,73. 2% respectively,while in the combined dection group detection,specificity,sensitivity and accuracy rates were 89. 2%,60. 5% and 77. 03 %,higher than the individual inspection teams group ROC curve analysis showed that the largest detection area was up to 0. 765 in combined dection group,more than the individual test groups. Positive rate,compared with the individual test,was higher than the single detection,with statistically significant difference. Conclusions The combined detection of serum GP73,AFP,AFP-L3 and AFU helps to improve the detection rate of primary liver cancer and liver cirrhosis,chronic hepatitis phase identification.
