摘 要:目的探讨肿瘤标志物癌胚抗原(CEA)、甲胎蛋白(AFP)、糖类抗原19-9(CA19-9)、糖类抗原125(CA125)、人绒毛膜促性腺激素(β-HCG)联合检测比较单项检测对卵巢肿瘤的诊断更有价值。方法应用化学发光免疫分析法分别检测实验组恶性肿瘤患者106例,良性组患者184例,健康对照组50例血清标本,通过联合检测CEA、AFP、CA 19-9、CA125、β-HCG 5项肿瘤标志物,同步比较单项数据,比较敏感度、特异度及准确度。结果恶性卵巢肿瘤组CEA、AFP、CA19-9、CA125、β-HCG单项检测含量明显高于良性卵巢肿瘤组、健康对照组,差异有统计学意义(P<0.01);恶性卵巢肿瘤组血清CEA、AFP、CA 19-9、CA125、β-HCG阳性率分别为10.38%、8.49%、32.08%、73.58%、6.60%,恶性卵巢肿瘤组的5项联合检测阳性率高达90.57%,两者比较差异有统计学意义(P<0.05);恶性卵巢肿瘤组血清CEA、AFP、CA 19-9、CA125、β-HCG准确度分别为71.18%、70.88%、75.00%、86.18%、69.41%,5者联合检测恶性肿瘤准确度高达88.24%。结论 CEA、AFP、CA19-9、CA125及β-HCG联合检测有助于提高诊断的敏感度和准确度,是诊断女性卵巢恶性肿瘤理想的检测指标。Objective To investigate whether combined detection of CEA, AFP,CA19-9,CA125 and 13-HCG is more valuable than single detection in ovarian tumor diagnosis. Methods The sensitivity and specificity was compared between combined diagno-sis by detecting 5 kinds of tumor markers (CEA, AFP,CA19-9, CA125,β-HCG) through Chemiluminescent immunoassay and sin-gle detection of ovarian tumor diagnosis in three groups (malignant tumors patients group, benign group and the healthy control group). The number of malignant tumors group, benign group and the healthy control group was respectively 106,184,50. Results There was a significant difference of single indicator CEA, AFP,CA19-9,CA125, β-HCG in ovarian cancer group, benign group and the healthy control group, difference was statistically significant (P〈0.01). The sngle positive rate of CEA, AFP, CA19-9, CA125, β-HCG in ovarian carcinoma serum were 10.38%、0.8.49%、 32.08 %, 73.58 %、 6.60% respectively. The positive rates of combined detection of CEA,AFP,CA19-9,CA125,β-HCG in ovarian carcinoma was up to 90.57 %, which was significantly higher than the single diagnosis, difference was statistically significant (P〈0.05). The single accuracy of CEA, AFP, CA19-9,CA125, β-HCG in o-varian carcinoma serum were 71.18%、70.88%、75.00%、86. 18%、69.41% respectively,while the combined detection was up to 88.24%. Conclasion The combined detection of tumor markers can help to improve the diagnostic sensitivity and specificity and it is an ideal testing indicator for female ovarian tumor diagnosis.
