腰池持续引流治疗颅内动脉瘤夹闭术后蛛网膜下腔出血的临床效果分析

摘　　要：目的:探讨腰池持续引流术治疗颅内动脉瘤夹闭术后蛛网膜下腔出血的方法及效果。方法:20例颅内动脉瘤夹闭术后蛛网膜下腔出血患者行腰池持续引流作为治疗组,另外20例行间断腰穿放液治疗作为对照组,进行疗效观察,所获数据经X2,t检验。结果:治疗组与对照组脑脊液中红细胞消失时间分别为(4±1.5)天和(8±2.1)天(P<0.001),两组CSF中总蛋白(TP)<0.8g/L时间分别为伤后(6±2.7)d和(11±1.8)d(P<0.001),引流3天后颅内压增高发生率分别为l2.45%和38.63%(P<0.01)。治疗组头痛、脑膜刺激征持续时间短,脑积水及脑梗塞发生率、病死率均显著低于对照组。结论:腰池持续引流术治疗颅内动脉瘤夹闭术后蛛网膜下腔出血可使之尽早廓清,疗效确切。Objective：To study the method and efficacy of lumbar cistern continued drain age（LCCD）in the treatment of subarachnoid hemorrhage（SAH） after clipping of intracranial aneurysm.Methods：20 patients with SAH after clipping of intracranial aneurysm were treated with LCCD as therapeutic group（TG）,other 20 patients as control group（CG） were treated by intermittent lumbar puncture drainage,then observe the therapeutic efficacy and the data were treated with x2or t tests.Results：In TG and CG the time for erythrocyte elimination of cerebral spinal fluid were （4±1.5）and （8±2.1） days（P〈0.001）,time of total protein（TP）in CSF（〈0.8g/L） was （11±1.8）days and （6±2.7）days respectively〈0.001.The prognosis in TG were superior to CG（P〈0.0 5）.In TG,headache and neck rigidity restored quickly.Conclusion：LCCD for the treatment of SAH after clipping of intracranial aneurysm is a simple ,safe and effective way.
