经肿瘤周围与乳晕下注射超声造影剂检测乳腺癌前哨淋巴结的对比研究

摘　　要：目的对比分析经肿瘤周围与乳晕下注射超声造影剂检测乳腺癌前哨淋巴结(SLN)的应用。方法对70例乳腺癌患者分别经肿瘤周围与乳晕下注射造影剂,观察同侧腋窝SLN的造影增强情况并记录,与美蓝染色病理结果比较。结果以美蓝染色检出的SLN为标准,PT注射与SA注射检出率分别为86.3%、87%,假阴性率分别为3.3%、13.3%,两者检出SLN一致性较好(Kappa值=0.687)。结论经肿瘤周围与乳晕下注射超声造影剂均可以较好的评价乳腺癌SLN。Objective To discuss the effectiveness of contrast-enhanced ultrasound（CEUS） by using peritumoral and subareolar injection in detection of breast cancer sentinel lymph nodes（SLN）. Methods Seventy cases of breast cancer patients were treated with peritumoral（PT） and subareolar （SA）injection of contrast agents, the changes of CEUS of the ipsilateral axillary SLN were observed. The results were compared with methylene blue and surgicalbiop- sy. Results According to the subcutaneous injected methylene blue method, the identification rate of SLN was 86.3% in PT injection and 87% in SA injection,the false negative rate was 3.3% and 13.3%, the internal consistency of these two methods was good（Kappa value-0. 687）. Conclusions Both PT injection and SA injection are good method for evaluating prediction for breast cancer sentinel Lymph nodes.
