摘 要:目的通过对一例术中破裂前交通血流动力学特征的分析,探讨动脉瘤破裂相关的血流动力学危险因素。方法重建患者的三维影像资料,建立病例特异性模型,运用计算流体力学的方法对流场特征、瘤壁压力、壁面切应力(WSS)、剪切震荡指数(OSI)等进行量化分析。结果动脉瘤破裂点受到局部较高速度的血流冲击,具有低壁面切应力、高剪切震荡指数的特征,局部压力未见明显差异。结论动脉瘤破裂与血流冲击、低壁面切应力及高剪切震荡指数密切相关。Objective To investigate the hemodynamic risk factors of intracranial aneurysm rupture by analysis of an anterior communicating aneurysm ruptured during operation. Methods Computational fluid dynamic analysis was performed after reconstruction of the 3-dimensional rotated angiography (3DRA) data. Velocity, pressure of the aneurysmal wall, wall shear stress and oscillatory shear index were included in this study. Results The rupture site was located in the area which was impinged by the vortex in the aneurysmal sac. The rupture site had relatively lower wall shear stress (WSS) and higher oscillatory shear index (OSI). Pressure of the site had no significant difference with the peripheral area. Conclusion The rupture of intracranial aneurysm is closely related with the impingement of the vortex, lower WSS and higher OSI.
