摘  要:目的探讨p16、p21、p53和上皮细胞膜抗原(EMA)在单发性和多发性膀胱癌组织中的表达及意义。方法收集临床正常、单发和多发性膀胱癌标本,通过免疫组化法检测膀胱癌组织中相关基因的蛋白表达和定位分布。结果与正常膀胱黏膜上皮相比,p16和p53蛋白定位于细胞核,随着膀胱癌恶性程度的升高和侵袭程度的增加,p16蛋白表达明显下降,而p53表达明显升高(P<0.05)。p21蛋白定位于细胞膜,高分化膀胱癌的p21蛋白表达低于低分化膀胱癌(P<0.05);多发性膀胱癌表达阳性率明显高于单发性(P<0.05)。EMA主要表达于细胞浆,随着肿瘤分级和(或)分期的增加,阳性细胞数逐渐减少(P<0.05),在单发和多发性膀胱癌中的表达差异有统计学意义(P<0.05)。结论 p16、p53、p21和EMA在膀胱癌组织中不同水平的表达与膀胱癌的分级/分期密切相关,对膀胱癌相关基因的检测有助于膀胱癌的临床诊断、预后判断和治疗方法的选择。Objective To explore the expression and implicaitons of p16, p21, p53 and EMA in single and multiple bladder eance. Methods Samples of normal bladder, single and multiple bladder canc- er were collected,and the expression and locational distribution of p16, p21, p53 and EMA in these tissues were tested through immuno-bistoehemical technique. Results In contrast to normal bladder mucous mem- brane ,p16 and p53 mainly stayd in cell nucleus. With malignancy and infiltrating of bladder cancer increas- ing,the expression of p16 descended, while the expression of p53 ascended distinctly (P 〈 0. 05 ). p21 mainly stayed in cytoplasm ,the expression of p21 in well-differentiated bladder cancer was lower than poorly differentiated bladder cancer ( P 〈 0.05 ) , the positive rate of multiple bladder cancer was higher than that of single bladder cancer ( P 〈 0. 05 ). EMA stayed mainly in cytomembrane. With the grades and stages of tumors increasing, the positive cells decreased gradually ( P 〈 0. 05 ). The expression of EMA was signifi- cantly different between single and multiple bladder cancer ( P 〈 0. 05 ). Conclusions The different level expression of p16, p21, p53 and EMA in bladder cancer is closely related to the grades and stages of bladder cancer;Test of the relative gene of bladder cancer is helpful to clinical diagnosis, prognosis judgement and treatment selection.
