吉非替尼奥沙利铂联合氟尿嘧啶二线治疗晚期食管癌的疗效观察

摘　　要：目的观察吉非替尼、奥沙利铂联合氟尿嘧啶二线治疗晚期食管癌的近期疗效和不良反应。方法 52例晚期食管癌患者随机分为对照组(26例,奥沙利铂+氟尿嘧啶)和试验组(26例,吉非替尼+奥沙利铂+氟尿嘧啶),治疗4个周期后观察疗效及不良反应。结果对照组和试验组患者的有效率分别为34.6%和53.8%,疾病控制率分别为61.5%和84.6%,两组患者近期疗效比较,差异有统计学意义(P<0.05)。最常见的药物相关不良反应为骨髓抑制、恶心呕吐、皮疹和腹泻。与对照组比较,试验组患者的皮疹及腹泻发生率较高(P<0.05),但均为I^II级,对症治疗后均缓解。结论吉非替尼、奥沙利铂联合氟尿嘧啶二线治疗晚期食管癌的近期疗效优于奥沙利铂加氟尿嘧啶,且不良反应轻,患者耐受性好。Objective To observe the short-term efficacy and adverse effects of second-line treat- ment of gefitinib combined with oxaliplatin and fluorouracil for advanced esophageal cancer. Methods 52 ad- vanced esophageal cancer patients were divided into the control group （26 cases, oxaliplatin ＋ fluorouracil） and the experimental group （26 cases,gefitinib ＋ oxaliplatin ＋ fluorouracil）. The short-term efficacy and ad- verse reactions were evaluated after 4 cycles. Results The response rates（RR） of the control group and ex- perimental group were 34. 6% and 53. 8% ;the disease control rates（DCR） of the two groups were 61.5% a^M 84. 6% ;The experimental group had obvious advantages in short-term efficacy （P 〈 O. 05 ）. The most common adverse effects were myelosuppression, nausea and vomiting, drug rash and diarrhea. Rash and diarrhea inci- dence was higher in the experimental group than that in the control group. All adverse reactions were grade I or II and could be relieved after treatment. Conclusions The second line treatment of targeted drug gefitinib combined with oxaliplatin and fluorouracil for advanced esophageal cancer patients is better than the oxalipla- tin combined with fluorouracil in short-term efficacy, the adverse reactions were mild and well tolerated.
