腹腔镜与开腹Miles术治疗直肠癌的近期疗效分析

摘　　要：目的:探讨腹腔镜Miles术与开腹Miles术治疗直肠癌的近期临床疗效。方法:2011年8月至2013年6月为63例患者行直肠癌Miles术,其中腹腔镜组25例,开腹组38例,对比分析两组患者手术时间、术中失血量、淋巴结清扫数量、阳性淋巴结数量、肠功能恢复时间、住院时间、术后并发症等指标。结果:腹腔镜组术中出血量、肠功能恢复时间、住院时间、术后并发症均少于开腹组,差异有统计学意义(P<0.05),但手术时间多于开腹组(P<0.05)。淋巴结清扫数量、阳性淋巴结数量两组差异无统计学意义(P>0.05)。结论:与开腹Miles术相比,直肠癌腹腔镜Miles术具有患者创伤小、康复快、术后并发症少等优点,并可达到与开腹手术相似的根治效果,是安全、可行的。Objective：To assess short-term efficacy of laparoscopic versus open Miles operation in the treatment of rectal neo-plasm. Methods：Between Aug. 2011 and Jun. 2013,63 patients underwent Miles operation for rectal carcinoma. The patients were as-signed to laparoscopie group （ n = 25 ） and open group （ n = 38 ）. The operative time, blood loss, number of lymph nodes cleared, number of positive lymph nodes, time of intestine function recovered, hospital stay and postoperative complications were compared between the two groups. Results ：The blood loss, time of intestine function recovered, hospital stay and postoperative complications of laparoscopic group were significantly less than those of open group （P 〈 0.05）. But the operative time of laparoscopic group was significantly longer than that of open group （ P 〉 0.05 ）. The mean number of lymph nodes cleared and positive lymph nodes were not significantly different （ P 〉 0.05 ）. Conclusions ： The outcomes of this study suggest that laparoscopic Miles operation for rectal neoplasm has less trauma, fe-wer complications, faster recovery, and also has the same radical effect as open Miles operation. The laparoscopic Miles operation is safe and feasible.
