牙冠延长术在前牙外伤修复中的应用

摘　　要：目的评价牙冠延长术的临床应用效果。方法选择26例因外伤导致的单个上前牙冠折至龈缘的患者,行完善的根管治疗后,采用牙冠延长术再应用DMG纤维桩系统进行核桩修复及烤瓷冠修复。患者每6个月、1、2年复查,观察患牙、修复体和牙周情况,对修复效果进行评价。结果修复6个月、1、2年后复查时,患牙情况稳定,20颗患牙均无松动、修复体稳固、边缘密合,无牙龈炎、牙周炎、尖周炎发生,牙槽骨无明显吸收。6颗有轻度牙龈炎症,但无根周骨吸收,经洁治症状缓解。结论对于外伤所致的上前牙冠折至龈下的病例,采用牙冠延长术和纤维桩核进行桩冠修复可获得理想的修复效果。Objective To evaluated the clinical effect of crown lengthening on the fracture of anterior teeth. Methods A total of 26 military patients whose maxillary anterior teeth were fractured under gingivae were recruited. After completeness of root canal therapy, the teeth were treated by crown lengthening, and then with fiber post system. During the follow-up of 6, 12 and 24 months, periodontal conditions, radiographic signs and prosthodontic outcomes were recorded. Results The abutment and restora- tion were stable. There was neither dislodgment nor fracture in the posts and the roots, and the periodontal tissues were healthy. Conclusion Crown lengthening combined with fiber post system is a good treatment approach to the anterior teeth fractured under gingival.
