经颅多普勒联合颈动脉彩超检查在脑卒经颅多普勒患者中的应用

摘　　要：目的探讨经颅多普勒(transcranial doppler,TCD)联合颈动脉彩超检查在脑卒中高危患者中的应用价值。方法将广东省高州市人民医院收治的50例具有脑卒中高危因素的患者随机分为对照组和试验组,各25例。以TCD联合颈动脉彩超检查患者颅内外动脉狭窄或斑块的情况,对照组患者行常规抗血小板聚集治疗,试验组患者行常规抗血小板聚集+他汀类降血脂药物治疗,观察两组患者出院时的治疗效果及随访6个月发生(或再发)脑卒中情况。结果对照组有18例TCD及颈动脉彩超检查发现有颅内外动脉狭窄或斑块,试验组有20例,其发生(或再发)脑卒中的风险高,而使用他汀类降血脂药物可有效改善病情、降低风险,出院时对照组有效率为68%,试验组有效率为96%;随访6个月,对照组有9例发生(或再发)脑卒中,而试验组仅1例。结论对脑卒中高危患者使用TCD联合颈动脉彩超检查,能及时了解患者颅内外动脉的情况,为临床选择治疗方法提供客观依据,提高治疗效果,降低患者发生(或再发)脑卒中的风险。Objective To explore the application value of Transcranial Doppler （TCD） with carotid artery color Doppler ultrasound examination in patients at high risk of stroke. Method 50 patients were randomly divided into with high risk factors of stroke as the control group and the experimental group, each group 25 cases;TCD combined with carotid artery ultrasonography in patients with intracranial and extracranial artery stenosis or plaque, patients in control group received conventional antiplatelet therapy, the patients treated with conventional anti platelet aggregation and statins lipid lowering drug therapy, observed the effect of treatment and follow-up of discharged patients in the two groups was 6 months （or recurrent） stroke. Results In the control group had 18 cases of TCD and carotid artery ultrasonography was found to have intracranial and extracranial artery stenosis or plaque, the experimental group had 20 cases, the occurrence （or recurrent） risk of stroke is high, and the use of cholesterol lowering statin drugs can effectively improve the condition, reduce risk, control group at discharge efifciency 68%, the test group businesslike for 96%;And 6 months follow-up control group had 9 cases （or recurrent） stroke, while only 1 cases of test group. Conclusion Using TCD combined with carotid artery ultrasonography for patients at high risk of stroke, can timely understanding of the extracranial and intracranial arterial, choice of treatment method to provide an objective basis, improve the therapeutic effect, reduce the patients （or recurrent） risk of stroke.
