摘　　要：目的探讨Ⅳ期胃癌患者外周血CD4+T细胞和CD8+T细胞表达状态与疾病临床病理特征及预后之间的关系。方法采用流式细胞技术检测40例胃癌患者外周血CD4+T细胞和CD8+T细胞数目及二者之间的比值;收集胃癌患者临床病理资料并进行随访,分析CD4+T和CD8+T表达状态与临床病理特征及无疾病进展生存时间(PFS)之间的关系。结果 CD4+T细胞与胃癌患者年龄、肿瘤分化程度、KPS评分及PFS无明显相关性,与性别有相关性(P<0.05)。CD8+T细胞与性别、肿瘤分化程度有相关性(P<0.05),与年龄、Karnofsky功能状态评分(KPS评分)、PFS无明显相关性。CD4+/CD8+比值与性别、年龄、肿瘤分化程度均无相关性,与KPS评分显著相关(P<0.05)。生存分析发现,CD4+/CD8+比值与PFS有明显相关性(P<0.05)。结论 CD4+T细胞和CD8+T细胞在胃癌抗肿瘤免疫中发挥重要作用,检测其比值对于判断胃癌的预后具有一定价值。Objective To investigate the relationship of the presence of CD4^＋ , CD8^＋T cells in peripheral blood with the pathologic characteristics and prognosis of gastric cancer. Methods Floweytometry was used to analyze the number and proportion of CD4^＋, CD8^＋T cells in periphreral blood from 40 patients with gastric cancer. The clinical data of the pa- tients were collected. The relationship of the proportion and number of CD4^＋ , CD8^＋T cells with the pathologic data and prognosis were statistically analyzed. Results The number of CD4^＋T cells had no correlation with age, differentiation and KPS score. A higher number of CD4^＋T was detected in female patients （P〈0. 05）.The number of CD8^＋T cells was sig- nificantly associated with gender and differentiation （P〈0. 05 ） , and not associated with age and KPS score （P = 0. 15）. Both CD4^＋and CD8^＋T were not associated with the prognosis of gastric cancer. The ratio of CD4^＋/CD8~was significantly associa- ted with KPS score, while not associated with age, gender and differentiation. Those patients with normal ratio of CD4^＋/ CD8^＋ had a higher KPS score （ P〈 O. 05 ） and longer progression-free survival time （ P〈 0. 05 ）. Conclusions CD4^＋ and CD8^＋T cells may have an important role in anti-tumor immune of gastric cancer. Detection of these two kinds of cells has clinical value on evaluating the prognosis of gastric cancer.
