摘 要:目的调查住院患者营养风险发生率、营养支持率和支持方式,判断营养支持合理性。方法应用营养风险筛查(NRS2002)对大连医科大学附属第二医院2011年3月667例新入院患者进行调查,NRS2002≥3分为有营养风险,并调查住院期间营养支持使用情况。结果新住院患者NRS2002≥3分的有108例(16.2%),使用营养支持的有17例(15.7%);其中肠内营养(EN)3例(17.6%),肠外营养(PN)11例(64.7%),EN+PN 3例(17.6%)。NRS2002<3分者中无使用营养支持者。普外科住院患者7 d后复筛,新增NRS2002≥3分者22例,其中使用营养支持16例(72.7%),EN 6例(37.5%),PN 4例(25.0%),PN+EN 6例(37.5%)。所有营养支持中PN与EN的比例是1.7∶1。结论存在营养风险的患者中只有少数得到营养支持,且营养支持不规范之处较多。Objective To investigate the prevalence rate of nutritional risk,the application of nutrition support and the methods of nutritional support,and to judge the rationality of nutritional therapy.Methods Nutritional risk screening(NRS2002) was applied to investigate on 667 cases of newly admitted patients in Second Affiliated Hospital of Dalian Medical University in March 2011,NRS2002 ≥ 3 was set as nutrition risk,and the use of nutritional support during hospitalization was investigated.Results 108 fresh in-patients (16.2%) had an NRS≥3.17 patients received nutritional support(15.7%),3 of whom received enteral nutrition(EN) (17.6%),11 received parenteral nutrition (PN) (64.7%),and 3 received EN + PN (17.6%).None of the patients whose NRS 〈 3 accetped nutritional support.Rescreening was performed in General surgery department 7days later.Another 22 patients with NRS ≥ 3 emerged,16 of them received nutrition support(72.7%),including 6 cases of EN(37.5%),4 case of PN(25%),and 6 cases of PN +EN (37.5%).The average PN and EN ratio was 1.7∶ 1.Conclusion Only minor part of the hospitalized patients under nutrition risk receive nutrition support,and there are numerous unstandardized aspects of nutrition support in use.
