腹腔镜下行肝左外叶切除治疗原发性肝癌的临床疗效分析

摘　　要：【目的】探讨腹腔镜下进行肝左外叶切除治疗原发性肝癌的临床效果。【方法】分析48例本院2010年3月至2012年12月收治的原发性肝癌患者的临床资料，根据手术方式将患者分为观察组和对照组。观察组26例，采用腹腔镜下肝左外叶切除术治疗；对照组22例，采用开腹手术治疗。观察并比较两组患者的手术时间、术中出血量、术后平均住院时间以及术后的并发症情况。【结果】本组48例患者均成功完成手术，观察组患者的手术时间以及术后平均住院时间均明显短于对照组，术中出血量少于对照组，且两组相比较差异有显著性（ P＜0．05）；观察组患者无明显的术后并发症，对照组患者中有2例患者术后出现腹水。【结论】腹腔镜下行左外叶切除术用于治疗原发性肝癌临床效果显著，可有效缩短手术时间，安全性高，术后并发症少，对提高患者预后质量具有重要的临床意义。[Objective]To explore the clinical efficacy of laparoscopic liver left lateral and external lobectomy for the treatment of primary hepatic carcinoma .[Methods]Clinical data of patients with primary liver cancer in our hos-pital from March 2010 to Dec .2012 were analyzed .According to the operation mode ,all patients were divided into observation group and control group .The observation group（ n=26） underwent laparoscopic hepatic left lateral and external lobectomy ,while the control group（ n=22） underwent laparotomy .The operation time ,bleeding volume , postoperative hospitalization time and postoperative complications were observed and compared .[Results]All 48 pa-tients completed the operation successfully .The operation time and postoperative average hospitalization time in ob-servation group were shorter than those in control group ,but intraoperative bleeding volume was less than that in control group ,and there were significant differences （ P〈0 .05） .No obvious postoperative complications occurred in the observation groups ,while ascites occurred in 2 patients of the control group after the operation .[Conclusion]Laparoscopic left lateral and external lobectomy for the treatment of primary liver cancer has significant efficacy and can shorten the operation time .It also has high safety and less complication ,so it is of important significance for im-proving the prognosis and quality of life of patients .
