摘 要:目的观察不同能量和频率的铒激光牙体预备后牙髓组织中TGF-β1的表达,获得Er:YAG激光牙体预备的最佳参数。方法收集成年家犬切牙129颗(无分组),测得犬切牙唇侧颈部牙釉质与牙本质的厚度之和,根据相应比例,获得犬切牙唇侧颈部牙体预备的相应深度。不同能量和频率Er:YAG激光和高速手机分别行牙体预备,Ca(OH)2覆盖,术后72h固定、脱钙、包埋、切片,HE染色和免疫组织化学染色观察牙髓的变化。Image-pro Plus 6.0软件对免疫组化染色标本进行图像分析,得到各组图像的平均OD值。实验数据经SPSS17.0统计软件用单因素方差分析(One-way ANOVA)和LSD-t检验法分析其差异。结果 TGF-β1在犬牙髓组织中阳性表达定位于成牙本质细胞胞浆内及成牙本质细胞下层牙髓细胞和血管内皮细胞内。激光7.0W组平均光密度值显示最低值,激光8.0W组显示最高值,与其他组相比,差别有统计学意义(P<0.05)。激光7.5W组和高速手机组平均光密度值居中,二者之间无统计学差异(P>0.05)。结论设置适当参数的Er∶YAG激光可用于牙体硬组织的切割,不但切割效率较高,而且对牙髓组织的影响较小。Objective To observe the dental pulpal response and its biological effect to cut dental hard tissues in dog incisors by the Er:YAG laser and conventional high-speed handpiece,than get the best parameters of cutting dental hard tissues with Er:YAG laser,which caused minimal damage to dental pulp. Methods One hundred and twenty nine dog maxillary incisors with their roots and lingual surface amputated ,had their enamel/dentin thickness at buccal surface measured with a caliper,calculated the cutting thickness according to the corresponding proportions. All incisors of experiment dogs for the cutting of dental hard tissues by the Er:YAG laser with three different parameters and conventional high-speed handpiece were divided into four equal groups randomly (n=8). After 72 hours post-operation, all the teeth were extracted immediately and fixed in 4% paraformaldehyde for a week,then 10%EDTA for 3 months. The dental pulpal morphological and histological changes for TGF-131 were evaluated. Average optical density (OD) values of all immunohistochemical images and results of HE staining were measured with Image-Pro Plus 6.0 image analysis software.The data were analyzed statistically by oneway ANOVA and LSD-t test (P=O.05) with SPSS 17.0. Results Immunohistochemistric examination showed that TGF-β1 located in the cytoplasm of odontoblast and pulp cells. There was no statistically significant difference in expression of TGF-β1 between the group of 7.5W output and conventional high- speed handpieee. Compared with other groups, the expression of TGF-β1 in dental pulpal tissue was fewest pronounced in the group of 7.0W, and the expression of TGF-β1 in odontoblast was significantly more pronounced in the group of 8.0W output, and there was statistically significant difference with other groups. Conclusion Er:YAG laser with 7.0W output causes no pulpal damage and appears to be a safe method for cutting dental hard tissues.
