不同近视度数眼高阶像差在调节过程中的变化

摘　　要：目的 研究不同近视程度患者在调节过程中高阶像差的变化及其影响因素.方法 采用基于Harmman-Shack 原理设计的Wavescan波前像差仪,在自然瞳孔状态下,分别测量65眼在0～5.0 D（步进1.0 D）调节刺激下的的波前像差,比较低度、中度、高度近视组的高阶像差在调节中的变化,并提取固定3.0mm瞳孔直径下的各阶像差.结果 调节放松时,高阶像差RMS值与近视度数之间没有相关性,总高阶像差、球差调节的变化趋势随着近视度数的增加而增加.3组的瞳孔直径均随调节反应的增加而减小.结论 人眼调节过程中,高阶像差、球差、彗差均发生改变,且这种变化趋势随着近视度数的增加而增加,高度近视眼在调节过程中视网膜像质可能会降低.Objective To investigate the changes of high-order aberrations （HOAs） in patients with different myopic degrees during accommodation and the related influential factors. Methods 65 eyes were enrolled;accommodative stimulus were in the range from 0 to 5 D（with an increment of 1.0 D） ;Hartmann-Shack aberrometer was applied to the measurement of the wavefmnt aberrations over natural pupil size;a comparison was made to the changes in aberrations during accommodation in low myopia group（ Group 1, n = 23, SE ： - 0.50 to - 3.00 D, mean ＋ SD： - 2.352 ＋ 0. 868 D）, moderate myopia group （ Group 2, n = 23, SE： - 3.25 to - 6.00 D, mean ＋ SD： -4.813 ＋0.900 D） ,high myopia group and under different pupil conditions（ natural and fixed 3.0 mm pupil）. Results Under relaxed condition, there existed no correlation between root-mean-square （RMS）of HOAs （HORMS）and myopic degrees ; HOAs and coma-aberrations increased during accommodation along with the increase of myopic degrees; the pupil size decreased during accommodation（ r = - 0.324 to - 0.437, respectively, P 〈 0.01 ）. Conclusions The total high-order aberration, spherical aberration and coma aberration changed during accommodation, and the trend of alternation increased with the increase of myopia degrees;retinal image quality in patients with high myopia degree may decrease in accommodation.
