中浆患者黄斑区形态、视功能与A型行为评分的相关研究

摘　　要：目的探讨中心性浆液性脉络膜视网膜病变(CSC)患者A型行为问卷评分、黄斑区神经上皮层脱离高度、视网膜功能之间的相关性。方法选取CSC患者40例(40只眼)。应用A型行为问卷、光学相干断层扫描(OCT)和多焦视网膜电图(mfERG)进行检查。对上述数据作Spearman相关分析。结果 CSC患者A型行为问卷评分(33.80±6.36)与最佳矫正视力(BCVA)呈负相关(r=-0.515,P<0.01),A型行为问卷评分与黄斑区神经上皮层脱离高度(302.42±71.62)μm呈正相关(r=0.378,P<0.05),BCVA与黄斑区神经上皮层脱离高度呈负相关(r=-0.738,P<0.001),mfERG P1波1、2环的反应密度与A型行为问卷评分呈负相关(P<0.05),mfERG P1波1、2环的反应密度与BCVA呈正相关(P<0.01),mfERG P1波1环潜伏期与BCVA呈负相关(P<0.01),mfERG P1波1、2环的反应密度与黄斑区神经上皮层脱离高度呈负相关(P<0.01),mfERG P1波1环潜伏期与黄斑区神经上皮层脱离高度呈正相关(P<0.01)。结论 A型行为与CSC之间存在着密切联系。矫正A型行为者的不良行为特征对CSC的预防甚至治疗都将具有重要的意义。[ Objective ] To investigate the correlation between the score of type A behavior, the height of neurosensory retinal detachment in the macular and retinal function in patients with central serous chorioretinopathy （CSC）. [Methods] The type A behavior pattern questionnaire, optical coherence tomography （OCT） and multifocal electroretinography（mfERG） were performed in forty patients （40 eyes） with CSC. Statistical analyses were performed by Spearman related analyses. [Results] There was negative correlation between the score of type A behavior （33.80±6.36） and the best visual acuity （BCVA）（r =-0.515, P 〈0.01）. There was positive correlation between the score of type A behavior and the height （302.42±71.62） μm of neurosensory retinal detachment in the macular （r = 0.378, P 〈0.05）. There was negative correlation between BCVA and the height of neurosensory retinal detachment in the macular （r =-0.738, P 〈0.001）. There was negative correlation between mfERG P1 amplitude density of rings 1, 2 and the score of type A behavior （P 〈0.05）. There was positive correlation between mfERG P1 amplitude density of rings 1, 2 and BCVA （P 〈0.01）. There was negative correlation between mfERG P1 latency of ring 1 and BCVA （P 〈 0.01）. There was negative correlation between mfERG P1 amplitude density of rings 1, 2 and the height of neurosen- sory retinal detachment in the macular （P 〈0.01）. There was positive correlation between mfERG P1 latency of ring 1 and the height of neurosensory retinal detachment in the macular （P 〈0.01）. [ Conclusions] There is a close rela- tionship between type A behavior and CSC. Rectifying the ill performance characteristics of people with type A be-havior will have important significance for the prevention and even treatment of CSC.
