抑郁症患者血清甲状腺激素相关因素分析

摘　　要：目的探讨影响抑郁症患者血清甲状腺相关激素(T3、T4、TSH)水平的相关因素。方法采用化学发光免疫法检测56例抑郁症患者(研究组)及与其人口学资料相匹配的30例健康者(对照组)血清三碘甲状腺原氨酸(T3)、甲状腺素(T4)、及促甲状腺激素(TSH)水平,并进行比较;对研究组年龄、病程、性别、HAMD评定各因子(焦虑/躯体化、认知障碍、阻滞、睡眠、体质量)进行Person相关分析。结果研究组血清甲状腺激素T3、T4均低于对照组(P<0.01),两组TSH比较无显著性差异(P>0.05);通过对研究组血清甲状腺激素水平Person相关分析发现T3水平与病程呈正相关(P<0.01),与HAMD评定中阻滞因子呈负相关(P<0.01);T4水平与抑郁症患者年龄、性别、病程、HAMD评定各因子无相关性(P>0.05);TSH水平与病程、HAMD总分呈正相关(P<0.01)。结论抑郁症患者存在甲状腺功能异常,与病程、HAMD总分、阻滞因子相关。Objective To explore the abnormality and influencing factors of level of serum thyroid related hormones （T3,1＇4, TSH） in patients with depression. Methods Levels of T3, T4 and TSH were measured by using chemil immunoassay in 56 depressive patients （study group） and well-matched 30 healthy controls （control group）. Person correlation analysis was performed to detect the correlation between level of serum thyroid hormone and age, disease course, gender, score of Hamilton Depression Scale （HAMD） in study group. Results Levels of serum T3 , T4 in study group were significantly lower than those in control group （P 〈 0.01 ）, and there was no significant difference in level of serum TSH between the two groups （P 〉 0.05 ）. Person correlation analysis showed that level of T3 in study group was significantly positively correlated with course of disease （P 〈 0.01 ） and was significantly negatively correlated with factor score of inhibition of HAMD （P 〈 0.01 ）. There was no significant correlation between level of T4 and age, or gender, or course of disease, or result of HAMD （ P 〉 0.05 ）. Level of TSH in study group was significantly positively correlated with course of disease and total score of HAMD （ P 〈 0. 01 ）. Conclusion Depressive patients have abnormalities in level of serum thyroid hormone,, which are correlated with course of disease, total score of HAMD and factor score of inhibition of HAMD.
