血清CA19-9及相关指标检测在胰腺癌中ROC曲线及相关性分析

摘　　要：目的探讨血清CA19-9在胰腺癌诊断中的临床意义。方法选择66例胰腺癌患者和85例胰腺正常患者,测定血清CA19-9等肿瘤标志物和肝功能各项生化指标,并结合临床资料进行分析。结果胰腺癌组患者血清CA19-9水平明显高于胰腺正常组(P<0.05);血清CA19-9升高与患者肝脏损伤及血清CA125有一定相关性;黄疸患者CA19-9水平显著高于无黄疸患者(P<0.05);血清CA19-9水平与胰腺癌肿瘤及患者体质量减轻程度有关;血清CA19-9水平对伴有黄疸的胰腺癌患者诊断价值高于无黄疸患者。结论血清CA19-9在胰腺癌诊断中有重要的临床价值,与患者肝损伤和胰腺癌肿瘤负荷相关,根据血清胆红素水平将患者进一步分为黄疸与无黄疸患者可明显提高血清CA19-9对胰腺癌的诊断价值。[Objective] To investigate the value and correlation of the lever of CA19-9 in the serum for diagnosis of pancreatic cancers. [ Methods ] Concentration of serum tumor marker and serum biochemical parameter were de- termined in 66 patients with pancreatic cancers and 85 patients with benign disease, and the results were being ana- lyzed and evaluated by statistics. [Results] The lever of serum CA19-9 in diseases of pancreatic cancers was higher than benign disease （P 〈0.001）, and there were some relationship between serum CA19-9 and hepatic injuries in two groups. The level of serum CA19-9 in diseases of pancreatic cancers has a significant positive correlation with the tumor size and lose weight. The diagnostic value of the lever of serum CA19-9 in pancreatic cancers was better in jaundice patients. [ Conclusions] Serum CA19-9 could be an important index on pancreatic cancers, and it has relationship with hepatic injuries and tumor burden, according to whether or not jaundice patients, the value of diag- nosis of pancreatic cancers would have significantly improved.
