ProTaper机用镍钛器械预备弯曲根管的临床评价

摘　　要：目的观察ProTaper机用镍钛器械预备弯曲根管的临床效果。方法 180例患者分为两组,每组患牙90个。对照组患牙用不锈钢K锉以标准的逐步深入法进行根管预备;实验组患牙用镍钛机用器械以冠根向深入法进行根管预备。拍摄X线片,观察预备后根管的形态及根管充填效果;记录根管预备后器械折断和变形情况。1周后复诊,评价患牙的疼痛发生率。结果根管预备时间实验组为(3.68±0.82)min,对照组为(10.38±0.32)min;预备后根管不良形态的发生率,对照组为9.32%,实验组为2.30%;根充适充率,对照组为80.51%,实验组为93.12%;根充后疼痛发生率,对照组为37.50%,实验组为10.00%。结论 ProTaper机用镍钛器械器械具有根管成型效果佳、恰充率高、根管预备后疼痛发生率低以及器械折断率低等优点,适用于弯曲根管的预备。[ Objective ] To investigate the clinical effects of ProTaper Ni-Ti rotary instruments in preparation of curved root canals. [Methods] One hundred and eighty root canals of 180 patients were equally divided into two groups at random. The cases in experimental group were instrumented by ProTaper Ni-Ti rotary instruments using ProTaper technique （NT group）; those in control group were instrumented by stainless steel hand K-files using Step- down technique （SS group）. The shape of root canalal and effects of obturation were evaluated with radio graphs dur- ing and after operation. The incidence of post-operative pain and the rate of instrument fracture were recorded. [Results] The preparation time was （3.68＋0.82） rain in experimental group and （8.38_＋0.32） min in control group. The rate of root canal deformity after preparation was 9.32% in control group and 2.30% 93.12% of the canals in experiment group were obsturated as compared with 80.51% in control group. The incidence of post-operative pain were 10.00% in Ni-Ti ruup while 37.50% in control group. [ Conclusions ] Under clinical conditions, ProTaper Ni- Ti rotary instruments can prepare root canal effectively and safely, the ProTaper Ni-Ti rotary instrumentation tech- nique is an efficient root canal preparation method for curved canals. But fracture of the instrument should be avoided.
