前房维持器和黏弹剂在白内障超声乳化吸出术中应用对角膜内皮细胞的早期影响

摘　　要：目的对比前房维持器和黏弹剂在白内障超声乳化吸出术中应用对术后角膜内皮细胞的早期影响。方法将年龄相关性白内障患者50眼随机分成两组,A组25眼采用前房维持器下超声乳化,B组25眼为对照组,行黏弹剂维持前房下超声乳化。采用非接触式角膜内皮镜于术前、术后1周、1个月对角膜内皮细胞进行随访观察,对角膜内皮细胞密度及形态学进行分析。结果两组间术后不同时间角膜内皮细胞丢失率:A组术后1周(24.02±18.38)%、术后1个月(28.01±16.00)%;B组术后1周(22.80±21.83)%、术后1个月(27.84±19.33)%;术后1周及1个月A、B两组角膜内皮细胞丢失率比较,差异均无统计学意义(P=0.832、0.973)。两组不同时间角膜内皮细胞变异系数的比较:A组术后1周(41.39±3.57)%及术后1个月(37.18±5.89)%均高于术前(36.80±5.75)%,B组术后1周(41.64±7.68)%及术后1个月(38.21±6.67)%均高于术前(36.92±5.57)%。两组不同时间中央角膜厚度:A组术后1周(525.92±39.69)μm及术后1个月(514.96±37.73)μm均高于术前(511.44±34.07)μm,B组术后1周(520.68±35.80)μm及术后1个月(504.96±32.28)μm均高于术前(497.00±30.43)μm。两组术后不同时间术前角膜内皮细胞变异系数及中央角膜厚度比较,差异均无统计学意义(均为P>0.05)。结论白内障超声乳化吸出手术中前房维持器持续维持前房并未加重角膜内皮细胞的损伤,说明白内障超声乳化吸出术中使用前房维持器是安全的。Objective To evaluate the early effects of anterior chamber maintai- ner and ophthalmic viscoelastic agent in phacoemulsification on the postoperative cor- neal endothelial cells. Methods Fifty eyes of age related cataract underwent phacoe- mulsification were included. Patients were randomly allocated into two groups of 25 eyes each. Cataract surgery was performed by phacoemulsification with anterior cham- ber maintainer in group A, and phacoemulsification was performed using ophthalmic viscoelastic agent in group B. The changes of corneal endothelial cell density and mor- phology were observed preoperatively and postoperatively at 1 week and 1 month. Re- suits The lose rate of corneal endothelial cells in group A at postoperative 1 week and 1 month were （24.02 ± 18.38）% and （28.01 ± 16.00）% , respectively, which in group B were （22.80 ±21.83）% and （27.84 ± 19.33）% , respectively, there was no statistical difference between two groups （P = 0. 832, 0.973 ）. In group A, variation coefficient of corneal endothelial cell at postoperative 1 week and 1 month were（41.39 -± 3.57）% and（37.18 ±5.89）% , they were all higher than that before operation（36.8 ±5.75）% ,and the central corneal thickness at post- operative 1 week and 1 month were（525.92 ±39.69） μm and（514.96 ±37.73） μm, they were all higher than that before op- eration（511.44 ± 34.07 ） μm.
