摘 要:3D腹腔镜的出现为外科手术带来了更为精准的操作。2013年11月至2014年1月南方医科大学珠江医院运用腹部医学图像三维可视化系统对16例患者CT数据进行三维重建和术前手术规划,并联合3D腹腔镜实施微创手术。其中胆囊切除术7例,肝脏肿瘤行肝切除术5例,术中联合胆道硬镜靶向碎石治疗肝胆管结石病3例,右肝巨大肝囊肿大部分切除术1例。16例患者术后无出血、胆汁漏等并发症发生。三维可视化技术联合3D腹腔镜,减少手术创伤,降低手术并发症发生率,临床应用前景良好。Three dimensional laparoscopy faciliated the precision manipulation in surgery. From November 2013 to Jan- uary 2014, the computed tomography data of 16 patients with hepatobiliary diseases were reconstructed by the abdominal medi- cal image three dimensional visualization software (MI-3DVS) to direct the operation. The operation was done under the three dimensional laparoseope. Of the 16 patients, 7 received laparo- scopic cholecystectomy, 5 with liver cancer received hepatecto- my, 3 with hepatolithiasis received pneumatic lithotripsy under rigid choledocboscope, 1 with large cyst in the right liver re- ceived subtotal resection of the liver. No postoperative bleeding and bile leakage were detected. Three dimensional visualization technology combined with three dimensional laparoscopy could reduce the incidence of surgical trauma and complications, and is helpful for achieving more accurate minimally invasive treatment.
