肾癌亚型的CT诊断与病理对照分析

摘　　要：目的:探讨肾癌各种亚型的CT表现,结合病理学特点分析,旨在提高诊断水平。方法:回顾性分析经手术病理证实的肾细胞癌61例,所有病例行螺旋CT平扫及多期增强扫描,CT表现与病理组织学相对照。结果:52例透明细胞癌呈圆形或类圆形30例,不规则形22例,平扫密度均匀或不均;其中15例见钙化灶,4例见出血灶,增强扫描大多数呈"快进快出"表现;2例乳头状细胞癌呈类圆形等密度影,增强扫描呈少血供表现,均匀轻度强化;4例嫌色细胞癌呈类圆形,其中2例病灶中央有钙化影,平扫密度较均匀,增强3例呈不均匀轻中度强化,1例见条索状强化;2例集合管癌类圆形或不规则形各1例,密度不均且1例病灶内见少量气体、钙化灶,增强均呈不均匀强化,并侵犯周围组织;1例多房囊性肾细胞癌呈类圆形低密度影,增强扫描分隔线实性部分明显强化。结论:肾癌的各种亚型CT表现有一定的特征性,螺旋CT动态增强扫描对术前诊断有帮助。Objective To analyze the CT appearance of renal cell carcinoma by different histological subtypes, combined with the analy- sis of pathological features ,designed to improve the diagnosis. Method Retrospective analysis of pathologically confirmed 61 cases of renal cell carcinoma,61 cases underwent MSCT scan and multi -phase enhanced scan, CT findings contrasted with histopathological. Results 52 cases of clear cell carcinoma were round or 30 cases were oval,22 cases were irregular shape, plain uniform or uneven density;15 cases saw calcification,4 cases saw hemorrhage, enhanced scan most was fast - forwardfaster performance ;2 casese of papillary cell carcinoma showed a round -like density shadow,enhanced scan showed hypovascular performance, uniform mild enhancement;4 cases of chromophobe RCC were round,including two eases of lesions in the centralcalcification scan more uniform density, enhanced three cases of mild to moderate enhancement was uneven, 1 case saw the cord -like enhancement;collecting duct carcinoma class round was 1 case irregular in shape was 1 case ,uneven density and one casesintralesional saw a small amount of gas, calcification, enhancement showed heterogeneous enhance- ment, and invade surrounding tissue;one case of multiple cystic renal cell carcinoma were round low density, the solid part of the enhanced scanning separator significantly enhanced. Conclusion CT findings of various subtypes of renal cell carcinoma have certain characteristics, dynamic contrast -enhanced spiral CT scan is helpful for preoperative diagnosis.
