摘  要:目的 利用静息态功能磁共振技术描述偏头痛患者大脑自发神经元的活动特征及功能改变,以期更好地认识偏头痛的发病机制.方法 收集16例头痛发作间期的偏头痛患者(病例组)和16名性别、年龄、受教育程度与其相匹配的健康对照(对照组),进行静息态功能磁共振成像扫描,采用DPARSFA 2.2软件分析原始数据,得出全脑低频振幅值,比较偏头痛患者头痛发作间期与健康人群之间影像学表现的差异.结果 病例组左侧岛叶、左侧额下回三角部、左侧额下回眶部低频振幅值(0.956±0.243)显著低于对照组(1.277±0.274;t=-3.88,P<0.01);与对照组相比,病例组右侧枕叶、右侧距状裂、右侧楔叶低频振幅值(1.081±0.237)显著高于对照组(0.774±0.140;t=4.33,P<0.01);病例组右侧枕中回低频振幅值和偏头痛患者的年发作频次呈负相关(r=-0.529,P<0.05).结论 偏头痛患者头痛发作间期疼痛调节相关的脑区功能异常,视觉皮质兴奋性增高,这有助于进一步理解偏头痛的发病机制.Objective We described the spontaneous neuronal activities and brain functional changes in patients with migraine for a better understanding of the pathogenesis.Methods Sixty patients with interictal migraine(case group) and 16 healthy volunteers with matched age,education and gender (control group) participated in this study.Using DPARSFA 2.2 software,we calculated the amplitude of low frequency fluctuation (ALFF) of the whole brain,and compared the different imaging findings between two groups.Results Relative to control group,the case group showed significantly decreased ALFF values (0.956 ± 0.243) in the left triangle and orbital section of inferior frontal gyrus,the left insula (1.277 ± 0.274 ;t =-3.88,P 〈 0.01),but with significantly increased ALFF values (1.081 ± 0.237) in the right occipital,calcarine and cuneus (0.774 ±0.140;t =4.33,P 〈0.01).In addition,we found that the ALFF values in the right middle occipital gyrus were negatively correlated with the frequency of migraine attacks (r =-0.529,P 〈 0.05).Conclusions The resting-state abnormalities of these regions may be associated with functional impairments in pain processing and increased excitability of the visual cortex in patients with migraine,which may help to understand the pathogenesis of migraine.
