摘 要:目的:探讨脂肪肝患者肝脏剪切波速度与实验室指标的相关性,为其研究提供可参考依据.方法:随机选择共纳入100例我院采用声触诊组织量化技术(virtual touch quantification,VTQ)行肝脏检查的脂肪肝患者及40例正常人群作为观察对象,记录所有受试者肝脏剪切波速度,且均空腹抽取静脉血检测实验室相关指标.结果:100例诊断为脂肪肝患者中,轻度脂肪肝46例,中度脂肪肝32例,重度脂肪肝22例.4组受试者剪切波速度、谷丙转氨酶(alanineaminotransferase,ALT),谷草转氨酶(aspartateaminotransferase,AST),高密度脂蛋白(high-density lipoprotein,HDL-C),低密度脂蛋白(low-density lipoprotein,LDL-C),总胆固醇(total cholesterol,TC),甘油三酯(triglycerides,TG)比较均有差异(P<0.05);其中,对照组与轻度脂肪肝组剪切波速度比较差异不明显(1.05m/s±0.09 m/s vs 1.09 m/s±0.12 m/s,P>0.05).中度脂肪肝肝脏剪切波速度最高(1.32 m/s±0.14 m/s),重度脂肪肝肝脏剪切波速度最低(0.85 m/s±0.07 m/s).轻中度脂肪肝剪切波速度与ALT、AST、HDL-C、LDL-C、TC、TG均存在密切相关性(均P<0.05),其中HDL-C为负相关关系,其余为正相关关系.结论:采用VTQ进行肝脏剪切波速度测定可以更为准确、客观的进行脂肪肝诊断与分度,其操作简便、耗时短;同时肝脏剪切波速度与肝功能及血脂等生化指标存在密切的相关性,VTQ值得临床推广应用.AIM: To explore the value of virtual touch tissue quantification (VTQ) measuring the liver shear wave speed and biochemical indicators in the quantitative diagnosis and classification of fatty live. METHODS: One hundred patients with fatty liver and 40 normal controls were included in the study. The shear wave speed of the liver was measured by VTQ, and alanine aminotransfer- ase (ALT), aspartate aminotransferase (AST), triglycerides (TG), high density lipoprotein (HDL-C) and low density lipoprotein (LDL-C) were measured. The relationship of these quan- titative indexes with fatty liver was analyzed.RESULTS: Among 100 patients with fatty liver, 46 had mild disease, 32 had moderate disease and 22 had severe disease. The values of shear wave speed, ALT, AST, TG, HDL-C and LDL-C were significantly different (P 〈 0.05 for all) between the control and fatty liver groups. The difference in the shear wave speed was not sig- nificant between the normal group and mild fatty liver group (1.05 m/s ± 0.09 m/s vs 1.09 m/s ± 0.12 m/s, P 〉 0.05). The shear wave speed was the highest in the moderate (1.32 m/s ± 0.14 m/s) and the lowest (0.85 m/s ± 0.07 m/s) in the serve fatty liver group. The shear wave speed in the mild-moderate fatty liver group was closely related with ALT, AST, HDL-C, LDL-C, TC and TG (P 〈 0.05 for all), of which HDL-C was negatively correlated with the shear wave speed while others were positively correlated with. CONCLUSION: VTQ and biochemical indicators have appreciable value in quantitative diagnosis of fatty liver and provide a reliable and objective basis for clinical diagnosis and treatment of this disease.
