硬化剂注射方法改良后对食管静脉曲张出血疗效及并发症的影响

摘　　要：目的:探讨硬化剂注射方法改良后对肝硬化食管静脉曲张出血的疗效及并发症的影响。方法:收集传统内镜下硬化剂注射治疗肝硬化食管静脉曲张出血258例次作为对照组,经改良的硬化剂注射方法治疗肝硬化静脉曲张出血268例次作为改良组,对比分析两组止血率、并发症发生率及并发症相关的病死率。结果:改良组止血率98.5%,对照组止血率97.6%,两组止血率相比无统计学差异;改良组并发症发生率17.2%,对照组并发症发生率43.4%,改良组并发症发生率明显低于对照组,且有统计学意义;改良组并发症相关病死率0.6%,对照组并发症相关病死率7%,改良组并发症相关的病死率较对照组显著下降,且有统计学意义。结论:内镜下硬化剂治疗肝硬化食管静脉曲张出血安全、有效,技术改良后操作更成熟,安全性更高,并发症更少。Objective ：Discuss of influence of modified sclerotherapy to the curative effect and complica-tions of cirrhosis esophageal varices bleeding . Methods：Collection of 258 cases of cirrhosis esophageal varices bleeding received our traditional endoscopic treatment as control group ;Improved group （intravenous injection） was to collect 268 cases of patients accepted modified sclerotherapy treat .The bleeding rate ,complications and mortali-ty related to complications are compared among the two groups .Results ：The hemostatic rate of improved group was 98 .5% ,in the meanwhile ,the hemostatic rate of control group was 97 .6% .There was no statistical difference be-tween the two groups ;The incidence of complications of the improved group was 17 .2% .Nevertheless ,the inci-dence of complications was 43 .4% in the control group ,which was obviously higher than that of the improved group ,with statistical significance ;Mortality related to complications of the improved group was 0 .6% ,which is much lower than that of the improved group and there was statistical significance as well .Conclusion ：endoscopic hardener treatment of cirrhosis esophageal varices bleeding is safe ,effective ,and reaveling more operational maturi-ty ,higher safety ,less complications after technology improvement .
