经直肠彩超在诊断前列腺肉瘤中的价值

摘　　要：目的 :探讨经直肠超声检查前列腺肉瘤的图像特点和内部血流变化。方法 :在适当膀胱充盈情况下经直肠探查前列腺 ,观察肿瘤大小、形态、内部回声、突入膀胱腔内的程度 ,彩超探测肿瘤内的血流分布、速度、阻力指数。结果 :7例前列腺肉瘤超声显示前列腺呈球形混合性肿块 ,表面不规则 ,呈菜花状向膀胱腔突出 ,肿瘤内血流供应丰富 ,平均血流速度为 34cm/ s,阻力指数 (RI)平均为 0 .78。结论 :经直肠彩超诊断前列腺肉瘤可靠性强 ,能显示整个肿瘤大小、内部回声、浸润程度、血流供应情况 ,有利于手术方法选择。Objective:To explore the transrectal ultrasonographic characteristics and hemodynamic changes of prostatic sarcoma.Methods:Seven patients with prostatic sarcoma were examined with transrectal color Doppler ultrasound under the condition of urinary bladder properly filled to observe the shape,dimension,internal echo of the tumor and the degree of the tumor protruding into the bladder lumen.The blood distribution within the tumors were observed under color Doppler sonography.Blood velocities and resistance indexes(RI)were measured with pulsed Doppler.Results:The tumors in all the 7 patients appeared spherically enlarged with irregular mixed echoes inside.Most of the tumors protruded into the urinary bladder lumen caulifloweredly.Color Doppler revealed abundant blood supply inside the tumor.The average velocity and RI were 34cm/s and 0 78 respectively.Conclusions:Transrectal color Doppler ultrasound which plays an important role in diagnosing prostatic sarcoma is of great use for surgeons to make operation decisions.
