摘  要:目的检测急性髓系白血病(AML)患者骨髓细胞中上皮钙黏着蛋白(E-cad)和β-连环蛋白(β-catenin)的基因表达水平,并探讨二者的临床意义。方法选取2010年9月—2012年7月南方医科大学南方医院血液科诊治的初发AML患者75例(AML组)及同期诊治的非恶性血液病患者20例(对照组)。应用实时定量PCR法分别检测两组患者骨髓细胞中E-cad和β-catenin mRNA的表达,比较AML组与对照组、AML组不同FAB分型间、AML合并髓外浸润组与无髓外浸润组、难治性AML组与非难治性AML组E-cad、β-catenin mRNA的表达水平,探讨二者基因表达水平与临床特征的关系。结果 AML组患者E-cad mRNA表达水平低于对照组〔0.003(0.007)与0.091(0.109)〕,而β-catenin mRNA表达水平则高于对照组〔0.026(0.311)与0.004(0.011);均P=0.000〕;AML组不同FAB分型间E-cad、β-catenin mRNA表达水平比较,差异均无统计学意义(P>0.05)。Spearman相关分析显示,E-cad mRNA表达水平与乳酸脱氢酶水平呈负相关(rs=-0.123,P=0.000),与患者年龄、初发时白细胞计数、外周血及骨髓原幼稚细胞比例均无线性相关(P>0.05);β-catenin mRNA表达水平与患者乳酸脱氢酶水平、骨髓原幼稚细胞比例呈正相关(rs=0.550,P=0.000;rs=0.699,P=0.000)。AML合并髓外浸润组患者E-cad mRNA表达水平低于无髓外浸润组〔0.001(0.001)与0.004(0.006),P<0.05〕;而β-catenin mRNA表达水平则高于无髓外浸润组〔0.140(0.152)与0.098(0.099),P<0.05〕。难治性AML组患者E-cad mRNA表达水平低于非难治性AML组〔0.002(0.002)与0.015(0.016),P<0.05〕,而β-catenin mRNA表达水平则高于非难治性AML组〔0.120(0.130)与0.045(0.045),P<0.05〕。结论 AML患者骨髓细胞中E-cad基因表达降低,而β-catenin基因表达升高;可能是白血病细胞E-cad表达缺失引起了β-catenin在细胞内聚集,进而导致白血病细胞恶性增殖、侵袭性增加。Objective To quantitatively detect the expression levels of E-cadherin(E-cad) and β-catenin in patients with acute myeloid leukemia(AML) and to analyze their clinical significance.Methods 75 AML patients(AML group) and 20 non-malignant hematological disease patients(control group) admitted to the Department of Hematology of Nanfang Hospital,Southern Medical Univerisity from September 2010 to July 2012 were selected.Real-time PCR method was used to detect E-cad and β-catenin mRNA expressions in bone marrow cells.The E-cad and β-catenin mRNA expression levels were compared between AML group and control group,AML patients with different FAB types,AML patients with or without extramedullary infiltration,refractory AML patients or non-refractory AML patients.The relationship between the mRNA expression levels and clinical features was also investigated.Results The E-cad mRNA expression level in the AML group was lower than that in the control group[0.003(0.007) vs.0.091(0.109)],but β-catenin mRNA expression level in the AML group was higher than in the control group[0.026(0.311) vs.0.004(0.011)](P=0.000).AML patients with different FAB types showed no statistically significant difference in E-cad and β-catenin mRNA expression levels(P〉0.05).Spearman correlation analysis showed that E-cad mRNA expression level was negative correlated with lactic dehydrogenase(rs=-0.123,P=0.000),but had no linear correlation with patients′ age,WBC count,proportion of peripheral blood and bone marrow blast cell(P〉0.05).β-catenin mRNA expression level was positively correlated with lactic dehydrogenase,proportion of bone marrow blast cell(rs=0.550,P=0.000;rs=0.699,P=0.000).E-cad mRNA expression level in AML patients with extramedullary infiltration was significantly lower than patients without extramedullary infiltration[0.001(0.001) vs.0.004(0.006),P〈0.05],but β-catenin mRNA expression level was significantly higher in AML patients with extramedullary infiltration
