摘　　要：目的探讨H型高血压对老年糖尿病性脑梗死预后的影响。方法将我院收治的70例老年糖尿病性脑梗死患者根据是否合并H型高血压分为观察组和对照组,观察组为H型高血压脑梗死合并糖尿病患者,对照组为单纯老年高血压糖尿病性脑梗死患者。2组均给予脑梗死对症治疗,采用NIHSS、mRS、BI评价2组患者预后情况。结果 2组治疗前收缩压、舒张压比较差异无统计学意义(P>0.05)。观察组治疗前Hcy浓度显著高于对照组,观察组治疗后Hcy略低于治疗前,观察组与对照组治疗前NISS、mRS、BI评分差异无统计学意义,对照组治疗90d后NISSH、mRS、BI评分显著优于观察组,观察组复发率显著高于对照组(P<0.05)。结论合并H型高血压的老年糖尿病性脑梗死患者神经功能缺损程度更加严重,预后不佳,高Hcy影响老年糖尿病性脑梗死患者预后的机制还有待进一步研究。Objective To study effect of the H blood pressure on the prognosis of elderly diabetic cerebral infarc tion. Methods 70 cases of elderly patients with diabetic cerebral infarction, according to with or without H hypertension, were divided into observation group and control group. Two groups were all given anti cerebral infarction treatment. According to the NIHSS, mRS, BI, prognosis of two groups of patients was evaluated. Results Between the observation group and control group before treatment,systolic pressure, diastolic blood pressure difference has no statistical significance （P〉 0.05）. Hcy concentration of observation group is significantly higher than that of the control group before treatment, after treatment Hcy of the observation group is slightly lower than that before the treatment. Between observation group and control group before treatment there was no statistically significant difference in NISS, mRS, BI score agents. In the control group after treatment of 90 d NISSH, mRS, BI score was significantly better than that of the observation group. Recurrence rate of the observation group is significantly higher than that of the control group. Conclusion Neurologic deficits degree of patients with elderly dia- betic cerebral infarction combined with H hypertension is more serious, and their prognosis is poor. High Hcy affects the prog- nosis of patients with senile diabetic cerebral infarction and its mechanism still needs further research
