阿加曲班颈动脉加压静滴治疗急性脑梗死40例疗效观察

摘　　要：目的探讨阿加曲班颈动脉加压滴注治疗急性脑梗死的临床疗效。方法选取我院2012-02-2013-02收治的急性脑梗死患者80例,随机分为观察组和对照组各40例。观察组采用阿加曲班颈动脉加压静滴治疗,观察组采用常规静脉注射等量阿加曲班治疗,比较2组临床疗效,根据治疗前、后2组患者的NIHSS评分评价治疗效果;根据治疗前后60dmRS评分评价患者的预后情况。结果治疗14d后,观察组总有效率90.0%,对照组70.0%,观察组临床疗效显著优于对照组,差异有统计学意义(χ2=5.00,P=0.019)。2组NIHSS评分较治疗前均显著下降(P<0.05),且观察组下降更为显著(P<0.05)。治疗60d后,2组mRS评分较治疗前显著下降(P<0.05),且观察组下降更为显著(P<0.05)。2组患者均未出现明显不良反应。结论阿加曲班颈动脉加压滴注治疗急性脑梗死安全有效,值得临床推广。Objective To analyze the clinical efficacy of carotid artery pressure drip of argatroban on acute cerebral infarc-tion.Methods 80 cases with acute cerebral infarction from February 2012 to February 2013 in our hospital were selected ,ran-domly divided them into control group and observation group ,each of the group with 40 cases. The cases in control group were given argatroban by intravenous infusion ,while those in observation group were given argatroban by carotid artery pressure drip. Clinical dataS of two groups including clinical efficacy ,NIHSS and mRs scores were analyzed.Results The total effective rate of observation group（90.0% ） was much more better that of control group（70.0% ）;the NIHSS scores and mRs scores of the two groups were significantly lower than those before treatment （P〈0.05） ,and it was much lower in observation group.and there was no obvious adverse reactions in both two groups.Conclusion Carotid artery pressure drip of argatroban is efficacious for patients with acute cerebral infarction and is safe. It is worth of clinical promotion.
