摘 要:目的:探讨脑白质改变(white matter changes ,WMC)与糖尿病、糖代谢异常的关系。方法回顾性分析2008-2012年杭州市第一人民医院神经内科477例脑白质改变患者的临床和影像学资料,采用Visser标准对脑白质改变严重程度评级,记录患者的临床资料,既往未诊断糖尿病而空腹血糖6.1~6.9 mmoL/L患者进行口服葡萄糖耐量试验(OG-TT),糖代谢分类采用2003年美国糖尿病学会建议标准。将脑白质改变(WMC)按分级与糖代谢异常进行相关性分析。结果血糖水平与脑白质改变严重程度有相关性(P<0.05)。结论脑白质改变严重程度与糖尿病、糖代谢异常密切相关。83%的脑白质改变患者合并糖尿病或糖耐量异常。Objective To investigate the association between diabetes mellitus and impaired glucose tolerance among the patients with cerebral white matter changes.Methods The clinical and imaging data of 477 patients with cerebral white matter changes in Neurology Department of the First People’s Hospital of Hangzhou were retrospectively analyzed between 2008 and 20l2.The associations between WMC and diabetes mellitus and impaired glucose tolerance were analyzed.The fasting plasma glucose level were tested when all the patients were hospitalized.The patients who were not diagnosed as diabetes with fasting plasma glucose level(6.1-6.9)mmol/L were examined by oral glucose tolerance test(OGTT).Results The incidence of WMC increased with plasma level of glucose.Conclusion WMC is associated with DM and OGTT.83.0% of patients with WMC are concomitant with diabetes mellitus or impaired glucose tolerance.
