重型颅脑损伤继发脑梗死临床特征及危险因素分析

摘　　要：目的分析重型颅脑损伤继发脑梗死临床特征及危险因素。方法收集2009-03—2013-06我院收治的157例重型颅脑损伤继发脑梗死患者的临床资料,分析重型颅脑损伤继发脑梗死临床特征并探讨患者的性别、年龄、GCS评分及有无脑挫裂伤、蛛网膜下腔出血、硬膜下血肿、低钠血症、低血压或休克及脑疝与继发脑梗死有无相关性。结果重型颅脑损伤继发脑梗死患者在性别和年龄两方面比较差异均无统计学意义(P>0.05);GCS评分、脑挫裂伤、蛛网膜下腔出血、硬膜下血肿、低钠血症、低血压或休克及脑疝比较差异均有统计学意义(P<0.05)。结论 GCS评分、脑挫裂伤、蛛网膜下腔出血、硬膜下血肿、低钠血症、低血压或休克及脑疝为重型颅脑损伤继发脑梗死的主要危险因素,可为有效指导医务人员及时采取措施提高重型颅脑损伤的救治率和改善预后提供理论支撑。Objective To analyze the clinical features and risk factors of infarction secondary to severe traumatic brain in- jury. Methods 157 patients with cerebral infarction secondary to severe traumatic brain injury were chosen, the clinical data were analyzed, the characteristics and the correlation of cerebral infarction with gender, age, GCS score and the presence of cerebral contusion, subarachnoid hemorrhage, subdural hematoma, hyponatremia, hypotension or shock and cerebral hernia- tion were explored. Results patients with cerebral infarction secondary to severe traumatic brain injury in both gender and age showed no significant difference, was not statistically significant （P〉0.05） ; in seven relevant factors , such as GCS score, cerebral contusion, subarachnoid hemorrhage, subdural hematoma, hyponatremia, hypotension or shock and herniation , the difference was statistically significant （P〈0.05）. Conclusion GCS score, cerebral contusion, subarachnoid hemorrhage, sub- dural hematoma, hyponatremia, hypotension or shock and herniation were the main risk factors of cerebral infarction secondary to severe traumatic brain injury. This conclusion can effectively guide the medical staff to take timely measures to improve the treatment of patients with severe traumatic brain injury and provide theoretical support to improve the prognosis.
