摘 要:目的:观察和分析三维适形放射治疗(IM RT )联合化疗治疗脑瘤的疗效。方法选取90例中晚期脑瘤患者作为研究对象,将其随机分为观察组和对照组各45例,对照组给予三联药物化疗进行治疗,观察组给予IM RT 联合化疗进行治疗。对2组患者的临床疗效进行评价和比较。结果观察组和对照组患者的临床总有效率分别为80%和37.8%,观察组显著高于对照组(P<0.05),且观察组中疗效为CR的患者比例显著高于对照组(P<0.05);照射剂量≥60 Gy的患者中疗效为CR、PR的比例显著高于照射剂量<40 Gy的患者(P<0.05),照射剂量40~60 Gy的患者中疗效为PR的比例显著高于照射剂量<40 Gy的患者(P<0.05);对照组中照射剂量≥60 Gy的患者中疗效为PR的比例显著高于照射剂量<40 Gy的患者(P<0.05)。结论应用IMRT联合化疗治疗脑瘤疗效显著,较高的照射剂量可收到较好的局部疗效,但为了降低治疗的不良反应,必须要选择适当的照射剂量。Objective To observe and analyze the curative effects of intensity-modulated radiation therapy (IMRT) com-bined with chemotherapy on patients with brain tumor.Methods 90 cases of patients with advanced brain tumors were selected as the research objects and randomly divided into observation group and control group. 45 cases each group. The patients in the control group were treated with triple chemotherapy treatment ,while the patients in the observation group were treated with IMRT combined with chemotherapy. The clinical effects of two groups were evaluated and compared.Results The clinical total effective rates of the observation group and the control group were 80% and 37.8% ,respectively. The efficacy of the observa-tion group was significantly higher than that of the control group (P<0.05) and the proportion of patients with the effect of CR in the observation group was significantly higher than that in the control group (P<0.05);in the observation group ,the proportions of the patients with CR or PR effects among the patients whose therapeutic dose ≥60 Gy were significantly higher than those of the patients whose therapeutic dose <40 Gy (P<0.05). The proportion of the patients with PR effect among the patients whose therapeutic dose were 40~60 Gy was significantly higher than that of the patients whose therapeutic dose <40 Gy (P<0.05);in the control group ,the proportion of the patients with PR effect in the patients whose therapeutic dose ≥60 Gy were significantly higher than those of the patients whose therapeutic dose <40 Gy (P<0.05).Conclusion The clinical effects of the application of IMRT combined with chemotherapy on patients with brain tumors are significantly better. A higher therapeutic dose can lead to better local effects. However ,in order to reduce the side effects of treatment ,the appropriate irra-diation dose should be selected.
