摘  要:目的探讨肌电生物反馈结合综合康复训练对偏瘫患者步行能力的影响。方法脑卒中偏瘫患者80例,随机分为对照组和治疗组各40例。两组患者均给予常规综合康复治疗,治疗组在此基础上给予患侧下肢胫前肌肌电生物反馈电刺激。两组患者在治疗前和治疗8周后进行Fugl-Meyer下肢运动功能、Fugl-Meyer平衡功能、踝关节主动活动度、踝关节最大背屈时胫前肌等长收缩时的肌电积分值(iEMG)以及Holden步行功能分级的评定。结果治疗8周后,两组患者的运动功能、平衡功能、踝关节主动活动度、胫前肌iEMG值、Holden步行功能分级比治疗前均显著提高(P<0.001),治疗组优于对照组(P<0.05)。结论在常规综合康复疗法的基础上结合生物反馈治疗技术,一定程度上可以提高患者踝关节运动功能控制能力,从而改善患者的步行能力。Objective To explore the effect of surface electromyography biofeedback combined with the routine comprehensive rehabili-tation treatment on walking ability of stroke patients with hemiplegia. Methods 80 stroke patients with hemiplegia were randomly divided into control group (n=40) and treatment group (n=40). All the patients received routine comprehensive rehabilitation treatment. The treat-ment group received electromyography biofeedback electrostimulation in addition. Fugl-Meyer Assessment was used to assess motor and balance function;active range of movement of ankle joint, the integrated electromyogram (iEMG) of isometric contraction of anterior tibia-lis muscle under maximum ankle dorsiflexion and Holden walking function were determined before and 8 weeks after treatment. Results 8 weeks after treatment, the scores of motor and balance function, the active range of movement of ankle, the iEMG, and Holden walking function significantly improved in both groups (P〈0.001), and improved more in the treatment group than in the control group (P〈0.05). Conclusion The electromyography biofeedback combined with routine comprehensive rehabilitation treatment can improve ankle motion control ability and walking ability of stroke patients with hemiplegia.
