摘  要:目的观察白内障超声乳化手术对假性剥脱综合征(pseudoexfoliation,PXF)患者角膜内皮的损伤程度。方法临床病例对照研究。将2011年2月至2013年7月在沈阳爱尔眼视光医院确诊的伴有假性剥脱综合征的自内障患者32例(32只眼)作为实验组,同期不伴有假性剥脱综合征的白内障患者37例(37只眼)作为对照组。使用非接触角膜内皮显微镜分别于术前,术后1个月、3个月测量角膜内皮细胞密度(corneal endothelia cell density,ECD)并计算角膜内皮细胞丢失率;使用Pen-tacam三维眼前段分析仪测量术前,术后1个月、3个月的角膜中央厚度(central corneal thickness,CCT),两组间进行比较。结果术前与术后1个月、3个月实验组平均ECD均较对照组少,差异有统计学意义(P〈0.05)。实验组平均CCT均较对照组大,但差异无统计学意义(P〉0.05)。术后1个月、3个月实验组角膜内皮细胞丢失百分率(%)较对照组大,差异有统计学意义(P〈0.05)。术后1个月实验组CCT值增加百分率(%)较对照组大,差异有统计学意义(P〈0.05)。结论假性剥脱综合征眼术前角膜内皮细胞密度较正常眼低,行白内障超声乳化手术术后的角膜内皮细胞丢失率比正常眼要大,术后短期角膜水肿较正常眼严重。Objective To compare corneal endothelial cell damage after cataract surgery in eyes with and without pseudoexfoliation (PXF). Methods From February 2011 to July 2013, 32 eyes with PXF after phacoemulsification were regarded as experimental group and 37 normal eyes as con- trol group. Preoperatively, 1 month and 3 months postoperatively, corneal endothelia cell density (ECD) and central corneal thickness (CCT)were measured by non-contact specular microscope and Pentacam HD. Results The mean ECD was significantly lower in the PXF group than in the non-PXF group preoperatively and postoperatively; the mean CCT was similar between groups throughout the follow-up period; however, 1 month and 3 months postoperatively, the percentage of endothelial cell loss was significantly greater in the PXF group than in the non-PXF group; the per- centage increase in CCT was significantly greater in the PXF group than in the non-PXF group 1 month postoperatively. Conclusions Corneal endothelial cell loss and transient in CCT are greater after cataract in eyes with PXF than in eyes without PXF.
