摘 要:目的评价视神经炎伴多发性硬化患者的眼球后血流动力学改变,并与健侧眼及健康对照组眼对比。方法回顾性分析34例单侧视神经炎伴多发性硬化的患者以及32名健康受试者。通过超声多普勒成像检查罹患视神经炎一侧眼的眼动脉、睫状后动脉和视网膜中央动脉的收缩峰值和舒张末血流速以及阻力指数。然后将这些数值与健侧眼以及健康受试者正常眼作比较。统计学方法分别采用配对t检验和非配对t检验。结果将视神经炎眼、对侧未累及眼以及健康对照眼,进行平均球后血流速度、平均阻力指数比较,差异无统计学意义(P〉0.05)。和对照眼相比,视神经炎眼的视网膜中央动脉的平均舒张末血流速度较低(P=0.04),视网膜中央动脉(P=0.02)和睫状后动脉(P=0.009)的平均阻力指数较高,其差异均有统计学意义P〈0.05。在患者有多发性硬化的患者中,和未累及的对侧眼相比,在睫状后动脉(P=0.02)和视网膜中央动脉(P=0.04)患有视神经炎侧眼具有更高的阻力指数,差异有统计学意义P〈0.05。球后血流速度和所有血管的阻力指数在未累及的对侧眼和正常对照眼差异无统计学意义(P〉0.05)。结论研究提示与对侧未累及眼以及健康对照眼相比,视神经炎伴多发性硬化与球后血流动力学受损相关,尤其在睫状后动脉和视网膜中央动脉。但是两者的因果关系有待于进一步论证。Objective To evaluate retrobulbar hemodynamics in patients with optic neuritis associ- ated with multiple sclerosis, and compare with the contralateral eye and the eyes of the healthy con- trol group. Methods From the list of neurology side neuritis recruit 34 patients with multiple scle- rosis and 32 healthy subjects. By ultrasound Doppler imaging optic neuritis side of the eye suffering from eye artery, posterior ciliary artery and central retinal artery peak systolic and diastolic blood flow velocity and resistance index (resistivity index). Then these values in healthy subjects with nor- mal contralateral eye and the eye were as comparison. Statistical methods were used paired t test and unpaired t test. Results The optic neuritis eyes, not involving the contralateral eye, as well as healthy control eyes, mean blood flow velocity after the ball, the average resistance index was no sig- nificant difference (P 〉0.05). And compared with control eyes, optic neuritis eye mean central retinal artery diastolic flow velocity was low (P =0.04), central retinal artery (P =0.02) and post- ciliary ar- tery (P =0.009), the average resistance index higher. In patients with multiple sclerosis patients, and not involving the contralateral eye compared ciliary artery (P =0.02), and central retinal artery (P = 0.04) with optic neuritis with higher resistance to lateral eye index. Retrobulbar blood flow velocity and vascular resistance index all without involving the contralateral eye and normal control eyes was no significant difference (P 〉0.05). Conclusions This study suggests that multiple sclerosis and optic neuritis is related with retrobulbar hemodynamics related damage, especially in the ciliary artery and central retinal arteries. However, a causal relationship between the two remains to be demonstrated.
