摘 要:目的评价实时二维超声技术在颅脑肿瘤外科手术中的应用价值。方法①28例颅脑肿瘤神经外科显微手术中应用实时超声技术,进行导航定位,选择皮层入路,实时指导肿瘤切除。②测量术中实时超声肿瘤体积并与术前MRI进行对比分析。结果①本组28例手术应用该技术定位精确,皮层切口减小,能清晰提供肿瘤动态切除图像;其中全切除25例(89.3%)、次全切除3例(10.7%);2例出现肌力短暂下降,2周后恢复至术前水平。②术中实时超声测得肿瘤体积值与术前MRI符合,P>0.05,差异无统计学意义。结论颅脑肿瘤手术中应用超声技术具有病灶定位快速准确、图像实时可靠等特点,通过术中动态扫描,有助于提高肿瘤的全切率和手术的安全性。Objective To evaluate the clinical value of the two-dimensional B-mode ultrasound system in brain tumor microsurgery. Methods Real-time ultrasound technology was used in 28 cases of brain tumor neurosurgery microsurgery for navigation and positioning and selecting approach and guiding tumor resection in real time. Intraoperative real-time ultrasound measurement of tumor size and the preoperative MRI were analyzed and compared. Results This cohort included 28 brain tumor cases. The total removal rate was 89.3% (25/28) and subtotal removal rate was 10.7 % (3/28). Muscle strength decreased was occurred in 2 cases, which resumed aiter two weeks. There was no statistical difference between intraoperative real-time ultrasound measurements and tumor preoperative magnetic resonance imaging (MRI), P〉0.05. Conclusions Intraoperative 2D real-time ultrasound system can localize lesions accurately, and is safe and reliable. Intraoperative ultrasound technique enables to perform a more radical tumor resection.
