摘　　要：目的探讨上海市某区近5年医患纠纷发生的基本情况,进而提出防控策略。方法应用Excel和SPSS 16.0软件对上海市某区近5年各级医疗机构上报的医患纠纷进行统计,采取定量与定性相结合的方法对其发生特点与原因进行分析,并提出防控医患纠纷的策略。结果某区2008—2012年间医患纠纷量呈逐年增多趋势,二、三级医疗机构发生医疗纠纷数较一级医疗机构多,纠纷多发场所为住院病房、门诊和急诊,多发科室为外科、内科、儿科,二级医疗机构医患纠纷涉及人员以初、中级职称为主、三级医疗机构以高级职称为主,纠纷投诉内容以诊疗问题为主,二、三级医疗机构纠纷涉及科室情况、纠纷涉及人员及纠纷投诉内容比较差异均有统计学意义(P<0.05)。结论医患纠纷发生的原因是多方面的,对各地区医患纠纷发生的具体原因与特点进行分析,采取相应的措施,才能有效地减少纠纷的发生。Objective To investigate the situation of medical disputes in one district of Shanghai in recent five years and to provide prevention strategy. Methods Data of medical disputes in one district of Shanghai in recent five years was an- . . （ alyzed by using Microsoft Excel and SPSS 16.0 software, and the charactensncs and causes of medical disputes were studied by using qualitative analysis and determined the nature analysis, and then the preventive strategy was proposed. Results The total rate of medical disputes had been increasing on a yearly basis from 2008 to 2012 in one district. The total rates of medical disputes in secondary and tertiary grade hospitals were higher compared with that in first grade hospitals; medical disputes happened mostly in sickroom, out-patient department, emergency department, surgical department, internal department and pediatrics department ; the staff involved in the disputes were mainly primary and intermediate professional titles holders in sec- ondary grade hospitals, while the staff in tertiary grade hospitals involved in the disputes mainly were senior professional title holders. The disputes mainly related to diagnosis and treatment. The differences in departments, staff and complaint matter in- volved in the disputes happened in secondary and tertiary grade were statistically significant （P 〈 0.05）. Conclusion The causes of medical disputes are varied. The disputes can be avoided by taking some corresponding measures after analyzing the specific causes and characteristics of disputes in each district.
