结直肠癌患者术前营养风险筛查与蛋白检测指标的相关性分析

摘　　要：目的通过对结直肠癌术前患者进行营养风险筛查(NRS)和蛋白测定,分析NRS与蛋白测定水平的相关性。方法使用NRS-2002对120例结直肠癌术前患者进行NRS,NRS评分≥3分者为有营养风险组,<3分者为无营养风险组;同时测定患者外周血白蛋白(ALB)、前白蛋白(PAB)和转铁蛋白(TRF)的水平。结果 120例结直肠癌术前患者中,NRS评分<3分者42例(35.0%),≥3分者78例(65.0%),营养风险与患者年龄和分期密切相关;有营养风险组与无营养风险组比较,ALB、PAB和TRF均有所降低,但仅PAB差异有统计学意义,ALB和TRF差异无统计学意义。结论结直肠癌术前存在一定数量的营养风险患者,年龄越大风险越大;NRS评分与ALB、PAB和TRF测定相关,尤其是PAB的测定,能较为敏感地反映患者的营养状况,为结直肠癌患者术前营养评估提供依据。Objective To assess the nutritional state of preoperative colorectal cancer patients and protein determination, ana- lyze the correlation between NRS and protein levels, and to find suitable protein detection index, provide the basis for clinical nutritional risk assessment and nutrition support. Methods The colorectal cancer patients were chosen in nutritional risk screening,this group had nutritional risk was defined as NRS -2002 score ≥ 3, another group had no nutritional risk was defined as NRS -2002 score 〈 3. Simul- taneously determined the peripheral blood albumin （ALB）, pre albumin （PAB） and transferrin （TRF） level in all patients. Results In 120 patients with colorectal cancer,NRS score 〈 3 points was in 42 patients （35.0%） ,ands≥3 points was in 78 cases （65.0%） ,Nu- tritional risk was correlated with age and stages. The comparison with nutritional risk group and the non - nutritional risk group, the level of ALB, PAB and TRF with nutritional risk group decreased, but only the level of PAB had significant difference. Conclusion Preopera- tive patients with colorectal cancer have a certain number of nutritional risk, the older the more risk ; The NRS score and ALB, PAB and TRF levels can sensitively reflect the nutritional status of the patients, and provide the basis for colorectal cancer patients with preopera- tive nutritional assessment.
