摘  要:目的 探讨乳腺癌成簇样微小钙化在乳腺X线片中的特征性表现。材料与方法 经手术病理证实并在乳腺X线片中可见微小钙化乳腺癌 1 0 8例 ,良性病变 5 0例。经 2位有经验放射科医生采用 7项指标分析微小钙化 ,判断病变良、恶性 ,运用 χ2 检验和Logistic回归方法评价各项指标对乳腺癌的诊断价值。结果 微小钙化判断乳腺癌最有价值的指标为 :( 1 )每平方厘米微小钙化数目 (N/S)大于 2 0个 ,总数目大于 30个。( 2 )钙化点间密度不均、大小不一。( 3)钙化形态为细沙型、混合型、蠕虫样或粗颗粒型。结论 乳腺X线片中微小钙化形态学表现、密度、大小及数目对乳腺癌的诊断具有重要价值。Objective To evaluate the clustered microcalcification on mammogram in the diagnosis of breast cancer.Materials and Methods Clustered calcification on mammogram was found in 108 cases with breast cancer and 50 cases with benign breast lesions, both were verified by pathology. An analysis of microcalcification with seven indexes was conducted by two experienced radiologists. Whether the lesion was malignant or benign was judged. The value of each index in the diagnosis of breast cancer was determined by using χ 2 test and Logistic multivariate stepwise regression analysis.Results The most useful characteristics of microcalcification for diagnosing breast cancer were: (1) the number of microcalcification per square centimetre (N/S) >20, and the total number >30; (2) heterogeneous density and different in size; (3) being presented as sand like, mixed, vermiform, or coarse granular patterns.Conclusion The morphological features, density, size and number of the microcalcification on mammogram are of great value for the diagnosis of breast cancer.
