结肠镜检查中腹部疼痛的评分对肠易激综合征的辅助诊断

摘　　要：目的：探讨在结肠镜检查中肠易激综合征（IBS）患者与非 IBS 功能性胃肠病（FGID）患者及健康对照腹部疼痛水平的差异，以了解结肠镜检查中腹部疼痛评分对肠易激综合征的辅助诊断作用。方法采用一般状况调查问卷、罗马Ⅲ诊断标准和疼痛视觉模拟评分（VAS），对2012年1月至8月来我院内镜室检查的149例 IBS 患者、56例非 IBS FGID 患者及77名健康对照进行结肠镜检查中疼痛感知水平的调查及评价。结果IBS 组与非 IBS 组（包括 FGID 组及健康对照组）疼痛评分[M（P 25～P 75）]分别为26（19～39.25）分和23（15～30）分，2组比较差异有统计学意义（P ＜0.01）；FGID 组及健康对照组疼痛评分分别为25（15～31.5）分和22（15.5～28.5）分，IBS 组与2组分别比较，差异均有统计学意义（P ＜0.01）。结论在结肠镜检查过程中，IBS 患者腹部疼痛感知水平高于非 IBS FGID 患者及健康对照；结肠镜检查中腹部疼痛水平评分可作为有效评价 IBS 患者内脏敏感性的指标，用以辅助诊断 IBS。Objective To study the differences in abdominal pain during colonoscopy between the IBS pa-tients,the non-IBS patients and the normal group,in order to understand the pain scores during colonoscopy and its value for diagnosis of IBS.Methods One hundred and forty-nine IBS patients,56 FGID but non-IBS patients and 77 healthy controls were included and examined by colonoscopy from January to August,2012.All people comple-ted the general status questionnaire,Rome-Ⅲ criteria questionnaires and pain scores on 0-100 mm visual analog scales after colonoscopy.Results The pain scores（median,interquartile range）of IBS patients and non-IBS pa-tients（including non-IBS FGID patients and healthy controls）were 26 （19-39.25）（P ＆lt;0.01）and 23 （15-30）（P ＆lt;0.01）respectively.The pain scores of IBS patients was higher than those of the healthy controls [（22 （15.5-28.5）,P ＆lt;0.01]or other FGID patients [25 （15-31.5）,P ＆lt;0.01].Conclusion The degree of abdominal pain during colonoscopy was higher in IBS patients than in non-IBS FGID patients and healthy controls.Colonoscopy using the VAS pain score can be useful in evaluating visceral sensitivity of IBS patients,and in identifying IBS pa-tients.
