侵及翼腭窝鼻咽血管纤维瘤的面中部掀翻摘除术

摘　　要：目的 :研究一种手术治疗侵及翼腭窝鼻咽纤维血管瘤的新术式—面中部掀翻摘除术。方法 :总结通过面中部掀翻 ,经鼻腔、上颌窦摘除侵及翼腭窝鼻咽纤维血管瘤 5例的临床经验。结果 :5例患者切口均一期愈合 ,无面部畸形 ,吞咽、发声功能正常。 1例术后出现面颊部血肿 ,1例术后发现一侧蝶窦有肿瘤残留。结论 :以术式面部无切口 ,术中视野广阔 ,出血少 ,可在直视下整体摘除肿瘤。术后患者恢复迅速 ,面部无手术瘢痕 ,无吞咽。Objective:To study a new method for excision of the nasopharyngeal fibrohemangioma extending into fossa pterygopalatina.Method:Five cases were surgical treated with midfacial deglove operation through nasal and maxillary.Result:The 5 cases all had incisions healed at the first stage without facial deformity, and they all got good swallow and vocal function while 1 case had facial hematoma postoperative and 1 case had residual carcinoma at one lateral of sphenoid sinus.Conclusion:This approach offers excellent exposure without leaving external facial deformity.The patients can recover soon without swallow and vocal block.
