摘  要:目的探讨JAK2V617F基因阳性与原发性血小板增多症患者发生血栓性疾病风险的关系。方法检索Pubmed、Embase、中国生物医学文献数据库数据库,以原发性血小板增多症JAK2V617F基因阳性患者与阴性患者发生血栓情况计算比值比(oddsratios,OR)和95%可信区间(confidenceinterval,CI),采用RevMan5.2软件和Statall.0软件进行荟萃分析和文献发表偏倚评估。结果共纳入23个研究,1710例JAK2V617F基因阳性患者和1263例阴性患者,采用随机效应模型进行分析,JAK2V617F基因阳性患者发生血栓性疾病的风险明显高于阴性患者(OR=3.13,95%CI:2.26~4.34)。亚组分析未发现结果亚洲人群(OR=4.98,95%CI:2.92~8.52)与非亚洲人群(OR=2.32,95%CI:1.62~3.33)之间存在差异。结论JAK2V617F基因阳性可明显增加原发性血小板增多症患者血栓性疾病发生的危险。Objective To investigate the association between the JAK2V617F gene and thrombosis in essential throm- bocythemia patients. Method The studies were identified by searching Chinese and English databases including Pubmed, Embase, Chinese Biomedical Literature Database and Wanfang Data. The Odds ratios (OR) and 95% confidence intervals (CI) were calculated using the rate of thrombosis in essential thromboeythemia patients with positive and negative JAK2V617F. The Meta analysis was conducted and bias in studies was evaluated by RevMan 5.2 and Stata 11.0 software. Result Twenty-three studies involving patients with essential thrombocythemia were included, and a total of 1710 JAK2V617F positive cases and 1263 negative cases were analyzed. JAK2V617F was associated with a significant increased risk of thrombosis (OR= 3.13, 95% CI: 2.26 - 4. 34) , while subgroup analysis showed no difference between Asia group (OR=4. 98, 95% CI: 2. 92 - 8.52) and non-Asia (OR=2.32, 95% CI: 1.62 - 3.33) by using random effects model.Conclusion JAK2V617F increases the risk of thrombosis in essential thrombocythemia patients significantly.
