摘 要:目的 探讨一氧化氮 (Nitricoxide,NO)在全胃肠外营养 (TotalParenteralNutrition ,TPN)导致正常肝脏及肝硬化大鼠肝损害中的影响。方法 30只正常Wistar大鼠和 30只肝硬化Wistar大鼠各自分别随机分为 5组 :A组 ,自由进食和水 ;B组 ,TPN ;C组 ,TPN +精氨酸 ;D组TPN +NG 硝基 L 精氨酸甲酯 (NG nitrio L argininemethylester,L NAME) ;E组 ,TPN +精氨酸 +L NAME。实验 7d后测肝功能、肝脂含量、肝脏NO含量、肝脏一氧化氮合酶 (NOsynthase ,NOS)活性和肝脏组织学检查。结果肝硬化大鼠肝脂和转氨酶B组比A组高 (P <0 0 5 ) ,C组比B组低 (P <0 0 5 ) ,D组比B组高 (P <0 0 5 ) ,D、E组较C组高 (P <0 0 5 ) ,E组较D组低 (P <0 0 5 )。正常大鼠各组间肝功能差异无显著意义 (P >0 0 5 ) ,正常组和肝硬化组的肝脂TG ,A组分别为 (6 9± 0 8) μmol/g、(9 1± 0 9) μmol/g ,D组为(4 9 7± 6 2 ) μmol/g、(34 2± 2 8) μmol/g ,C组为 (18 2± 4 4) μmol/g、(16 7± 2 7) μmol/g ,CHO变化与TG相一致 ,且与NO含量呈负相关。结论 NO能减轻TPN引起的肝损害。Objective[WT5”BZ] To investigate the effect of nitric oxide for hepatic injury induced by total parenteral nutrition(TPN) in normal and cirrhotic rats. [WT5”HZ]Methods[WT5”BZ] Thirty normal rats and thirty cirrhotic rats were randomly divided into five groups: Group A, chow and water at libitum;Group B, TPN only;Group C, TPN with additional arginine; Group D, TPN with N G nitrio L arginine methyl ester(L NAME, NO synthase inhibitor);Group E, TPN with arginine and L NAME. At the seventh day, liver function were examined and livers were resected in order to evaluate the lipid and nitric oxide content, nitric oxide synthase activity and histology.[WT5”HZ] Results[WT5”BZ] The hepatic lipid content and aminotransferase in cirrhotic group B was significantly higher than in group A( P <0 05), and that in group C was significantly lower than that in group B( P <0 05), while that in group D was significantly higher than in group B( P <0 05), that in group D and E were significantly higher than in group C( P <0 05).That in group E was significantly lower than that in group D ( P <0 05). Liver function was not significantly different among normal groups( P >0 05). The changes of hepatic lipid content in normal groups were consistent with that in cirrhotic groups, and were in negative correlation with the content of nitric oxide.[WT5”HZ] Conclusions[WT5”BZ] Nitric oxide production in liver might play a role in protection from liver injury induced by total parenteral nutrition in normal and cirrhotic rats.
