血甘油三酯对非酒精性脂肪性肝病患者纤溶酶元激活物抑制剂1及脂联素水平的影响

摘　　要：目的观察血甘油三酯(TG)对非酒精性脂肪性肝病(NAFLD)患者纤溶酶元激活物抑制剂1(PAI-1)、脂联素(APN)水平的影响。方法从山西医科大学第二医院内分泌科门诊就诊患者中经彩色超声诊断的脂肪肝患者中筛选符合条件者共24例进入研究,分为两组,即血甘油三酯正常组(NT组)、血甘油三酯升高组(HT组);以血TG、血尿酸、血糖正常的非脂肪肝人群作为对照组。测各组血清胰岛素、PAI-1、APN,以胰岛素抵抗指数(HOMA-IR)评价IR。结果与正常对照组比较,NT组PAI-1水平有所增高[(23.21±11.14)ng/ml vs(15.67±5.97)ng/ml],APN有所下降[(56.22±35.82)ng/ml vs(70.01±38.88)ng/ml],但差异均无统计学意义(P>0.05)。HT组PAI-1水平明显增高[(26.94±8.62)ng/ml vs(15.67±5.97)ng/ml],APN明显下降[(40.81±11.87)ng/ml vs(70.01±38.88)ng/ml],差异均有统计学意义(P<0.05)。结论 NAFLD患者PAI-1及APN水平受血甘油三酯影响。Objective To investigate the influence of serum triglycerides on plasminogen activator inhibitor-1 （ PAI- l） and adiponectin （APN） in patients with nonalcoholic fatty liver disease （NAFLD）. Methods Patients with fatty liver disease （n = 24） were selected and divided into two groups： normal serum triglycerides group （NT group） and hy- pertriglyceridemia group （HT group） , subjects without fatty liver disease were as normal control group. Serum insulin, PAI-1 and APN were measured and body weight, height were determined to assess body mass index （BMI） in fasting state. The PAI-1 and APN in NAFLD patients were analyzed, IR was indicated by HOMA-IR. Results PAI-1 was slightly increased [ （23.21 ± 11. 14） ng/ml vs （ 15.67 ± 5.97） ng/ml, P 〉 0.05] and APN was slightly decreased [ （56.22 ± 35.82） ng/ml vs （70.01 ± 38.88 ） ng/ml, P 〉 0.05 ] in NT group, but PAI-1 was significantly increased [ （26.94 ± 8.62） ng/ml vs （ 15.67 ± 5.97） ng/ml, P 〈 0.01 ] and APN was significantly decreased [ 40.81 ± 11.87 ） ng/ml vs （70. 01 ± 38.88） ng/ml, P 〈 0.05 ] in HT group compared with normal control group. Conclusion The levels of PAI-1 and APN are influenced by the levels of serum triglycerides in NAFLD patients.
