摘 要:目的:回顾性分析评价18F-FDG PET/CT显像对多发性骨髓瘤的诊断效能。方法:选择本院2011年3月-2013年6月临床可疑多发性骨髓瘤患者39例,其中男性22例,女性17例,年龄44-78岁,平均年龄(51.3±0.5)岁。均行PET/CT检查及临床骨穿或病理活检证实,分别统计其真假阳性例数及真假阴性例数,并计算灵敏度、特异性及准确性指标,评价诊断效能。结果:39例患者中真阳性18例,假阳性2例,真阴性16例,假阴性3例。PET/CT诊断灵敏度为85.7%(18/21),特异性为88.9%(16/18),准确性为87.2%(34/39),阳性预测值为90%(18/20),阴性预测值为84.2%(16/19)。结论:18F-FDG PET/CT可用于多发性骨髓瘤的诊断,结合患者临床表现及生化检查可提高诊断符合率。Objective: To evaluate the diagnostic accuracy of 18 F- FDG PET/CT imaging in multiple myeloma. Methods:All 39 patients with clinically suspected, MM were studied, male: female = 22: 17. Age was 44 -78 years old. All patients underwent 18F -FDG PET/CT imaging and medullary needle biopsy or operation and pathology. True or false positive and true or false negative were recorded and evaluated the diagnostic accuracy. Results:18 of 39 were resulted as MM in 18F -FDG PET/CT imaging, 16 of 18 were reviewed by medullary needle biopsy or operation and pathology,16 of 39 were not resulted as MM in 18F -FDG PET/CT imaging, 13 of 16 were reviewed by medullary needle biopsy or operation and pathology. The sensitivity, specificity, accuracy of diagnosis, positive or negative predictive of MM by 18F - FDG PET/CT imaging were 85.7% ( 18/21 ), 88.9% ( 16/18 ), 87.2% (34/39) ,90% (18/20) ,84.2% (16/19) respectivily. Conclusion: 18F - FDG PET/CT imaging has high ability of detecting MM. It will improve the diagnostic accuracy if we consider with clinical test.
