摘 要:目的探讨超声弹性应变率比值法与评分法对甲状腺良恶性结节的鉴别诊断价值。方法对150例患者的186个甲状腺结节分别进行超声弹性等级评分法及弹性应变率比值法分析,以术后病理结果为金标准,利用受试者工作特征曲线(ROC)比较两种方法的特异性、敏感性及准确性。结果超声弹性应变率比值法与弹性分级法计算甲状腺占位性病灶的ROC曲线下面积分别为0.917和0.896。两者判断甲状腺良、恶性病灶的敏感性、特异性、准确性分别为87.95%、78.89%、83.21%和88.66%、72.83%、75.37%,二者比较弹性比值法的特异性和准确性均高于弹性评分法,但两者差异均无统计学意义(P>0.05)。结论超声弹性应变率比值法与等级评分法都判断是甲状腺良、恶性占位性病变可行的新方法,其中超声弹性应变率比值法略优于等级评分法。Objective To compare the value of strain ratio method and classification method in differentiating benign and malignant thyroid nodules. Methods A total of 150 patients with 186 thyroid nodules were analyzed by elasticity score and elastic strain rate ratio,and its diagnostic value was evaluated by using ROC curves,and the sensitivity,specificity and accuracy were compared between the two methods. Results The area under ROC curve( Az) of ultrasonic elastic strain rate ratio method and elastic score to determine thyroid lesions were0. 917 and 0. 896,and the sensitivity,specificity accuracy of both two methods for differential diagnosis for thyroid benign and malignant lesions were 87. 95%,78. 89%,83. 21% and 88. 66%,72. 83%,75. 37%, respectively. In two methods,specificity,of the elasticity ratio method was higher than that of elastic grading,but the difference between them was no statistically significant( P > 0. 05). Conclusions Both of them can be the effective diagnostic standard in future,and the ultrasonic electrograph strain rate ratio method is a little better than the elastic classification method.
